Martin, Clerk'` REGULAR MEETING JUNE 26,_ 2012 The regular meeting of the Board of Public Works was convened at 9:39 a.m. on Tuesday, June 26, 2012, by Board President Gary A. Gilot, with Board Members Donald E. Inks, Kathryn E. Roos, and Michael C. Mecham present. Board Member Mark W. Neal was absent. Also present was Board Attorney Cheryl Greene. ADDITION TO THE AGENDA Mr. Gilot noted the addition to the agenda of a Request to Advertise for 2012 Streets and Sewers Materials. APPROVE MINUTES OF PREVIOUS MEETING Upon a motion made by Mr. Inks, seconded by Ms. Roos and carried, the minutes of the Agenda Review Meeting on June 7, 2012, and the Regular Meeting on June 12, 2012 were approved. OPENING OF QUOTATIONS — DESIGN AND INSTALLATION OF ETHERNET SERVICE FOR SIXTEEN (16) WATER WORKS FACILITIES — PR, OJECT NO. 112 -045_ .,(WATER _ WORKS) Mr. Gilot advised that this was the date set for the receiving and opening of sealed Quotations for the above referenced project. Ms. Greene noted that U.S. Signal did not submit the City Quote form. She stated this is really more of a request for proposals and she isn't sure how to read the numbers, or if either of the quotes is conforming to the specifications. She stated the names of the companies submitting quotes should be read and Legal and City staff should review them to determine if they are both responsive quotes. The names following Quotations were opened and read: AT &T 7720 Kennedy Avenue Hammond, Indiana 46323 Quotation was submitted by Mr. Keith Kroening Contractor's Non - Collusion Affidavit, Non - Debarment Affidavit, Employment Eligibility Verification, Non - Discrimination Commitment for Contractors and Certification of Use of United States Steel Products or Foundry Products was Submitted U.S. SIGNAL 506 West South Street South Bend, Indiana 46601 Quotation was submitted by Mr. Ralph Acito Contractor's Non - Collusion Affidavit, Non - Debarment Affidavit, Employment Eligibility Verification, Non - Discrimination Commitment for Contractors and Certification of Use of United States Steel Products or Foundry Products was Submitted Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the above Quotations were referred to Water, Wastewater, Legal, Engineering, and Purchasing for review, tabulation, and recommendation. OPENING — GOODSON COURT SEWER EXTENSION — PROJECT NO. 112 -035 SEWER INSURANCE REPAIR FUND Mr. Gilot advised that this was the date set for the receiving and opening of sealed Quotations for the above referenced project.
