NEW ITEM - PHASE 113: PE STAFF FOR OVERSIGHT & CONTRACTOR <br />Frost Engineering will provide one (1) professional engineer (Principal or <br />Engineer VIII) to monitor light demolition of area under review around 30" <br />main water line. The sub -contractor is Larson Danielson and their proposal is <br />estimated at 2-days, 8hr/day <br />For Larson Danielson: <br />The following is a scope for related demolition costs of the interior walkway and <br />excavation of soils within an enclosed space for engineering inspection of a supply pipe <br />for the South Bend Water Works Building. The not to exceed costs are provided for (2) <br />days of man power and equipment that require the excavation and demolition of existing <br />conditions for the soil erosion from the storm event. <br />1. Mobilize manpower, materials & equipment to complete the project in a professional <br />manner. <br />2. Furnish and install dust protection enclosure as required for area that requires saw <br />cutting of concrete. <br />3. Provide concrete slab sawing of 4'-0"x4'-0" existing area (thickness is assumed 6") to <br />gain access to examine conditions on other side for inspection by Frost Engineering. <br />4. Hand excavate materials as indicated underneath slab and 36" diameter pipe into <br />Contractor supplied dumpster. It is estimated (-)2'-8" from bottom of slab to top of pipe, <br />another 3'-0" of area around pipe, and additional 2'-0" to basement <br />floor for an overall height of 8'-0" +/- of enclosed space. <br />5. The man power for this project is figured for (1) superintendent and (3) workers for (2) <br />days based upon our site inspection on 01/04/19. Any additional days involved due to <br />cave in of soils within enclosed space would involve additional costs at a rate of <br />$2,550.00 per day. <br />6. Final clean up. <br />Clarifications: <br />• All work is figured for standard weekday working hours, 7:00am — 3:30pm. <br />• All work would be signed off by Engineer or City Project Manager on daily timesheets <br />and receipts. <br />• No work is configured to replace any material and replacement of concrete slab. <br />• Any additional work not described in this scope of work will be completed through <br />approved Change Orders. <br />• No permit fees or bonds are included in this proposal. <br />• The City of South Bend is required for location of utilities. <br />• All tools such as saws, jackhammer, barricades, lighting along with personal protection <br />equipment is provided by Larson Danielson. <br />• Time is configured for the safety training required by the Water Works Administration. <br />• Larson Danielson and sub -contractors will not be responsible for damage to unforeseen <br />underground obstructions not shown on drawings or located prior to the <br />commencement of work. Rerouting, relocation, disconnection, capping, and/or removal <br />of existing utilities would be additional if required. <br />• Barricades are to be provided by Larson Danielson for protection of work areas. <br />• (1) 20 cubic yard dumpster is figured for concrete materials and construction debris. <br />Any additional dumpster required would be additional to quote. <br />• Prices exclude taxes on materials. Please provide Tax Exemption Form for credit. <br />Important!
