<br />APPROVE CHANGE ORDER NO. 2 — RAVINA PARK -BOWMAN CREEK
<br />STABILIZATION — PROJECT NO. 117-073A (SPECIAL LOIT MAJOR PROJECTS)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 on behalf
<br />of Williams Creek Management Corporation, 4620 S. County Road 600 East, Plainfield, IN
<br />46138, indicating the contract amount be increased by $4,369.80 for a new contract sum,
<br />including this Change Order, in the amount of $249,269. Upon a motion made by Ms. Dorau,
<br />seconded by Ms. Fritzberg and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 4 — ERSKINE CLUBHOUSE RENOVATION — PROJECT
<br />NO. 116-108 (VPA MAINTENANCE FUND)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 4 on
<br />behalf of Ziolkowski Construction, Inc., 4060 Ralph Jones Drive, South Bend, IN 46628,
<br />indicating the contract amount be decreased by $15,400 for a new contract sum, including this
<br />Change Order, in the amount of $909,970. Upon a motion made by Ms. Dorau, seconded by Ms.
<br />Fritzberg and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 5 — ERSKINE CLUBHOUSE RENOVATION -PROJECT
<br />NO. 116-108 (VPA MAINTENANCE FUND)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 5 on
<br />behalf of Ziolkowski Construction, Inc., 4050 Ralph Jones Drive, South Bend, IN 46628,
<br />indicating the contract amount be increased by $14,214 for a new contract sum, including this
<br />Change Order, in the amount of $924,184. Upon a motion made by Ms. Dorau, seconded by
<br />Ms. Fritzberg and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 3 — LINCOLNWAY WEST AND CHARLES MARTIN
<br />SR. DRIVE IMPROVEMENTS — PROJECT NO. 117-072A (RWDA TIF)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 3 on
<br />behalf of Selge Construction Co., Inc., 2833 S. 111h Street, Niles, MI 49120, indicating the
<br />contract amount be increased by $9,353.50 for a new contract sum, including this Change Order,
<br />in the amount of $1,195,263.03 . Upon a motion made by Ms. Dorau, seconded by Ms. Fritzberg
<br />and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — ENGINEERING OFFICE IMPROVEMENTS —
<br />PROJECT NO. 118-107 (PROFESSIONAL SERVICES)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Ziolkowski Construction, Inc., 4050 Ralph Jones Drive, South Bend, IN 46628, indicating the
<br />contract amount be increased by $1,433 for a new contract sum, including this Change Order, in
<br />the amount of $9,233. Upon a motion made by Ms. Dorau, seconded by Ms. Fritzberg and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — OLIVE PRODUCTION WELL NO. 6
<br />REHABILITATION — PROJECT NO. 118-102 (WATER WORKS O&M)
<br />Mr. Gilot advised that Ms. Becca Plantz, Engineering, has submitted Change Order No. 1 on
<br />behalf of Peerless Midwest, Inc., 55860 Russell Industrial Parkway, Mishawaka, IN 46545,
<br />indicating the contract amount be increased by $12,307 for a new contract sum, including this
<br />Change Order, in the amount of $63,237. Ms. Kara Boyles, City Engineer, stated this project and
<br />the Edison Production Well Rehabilitation project are for cleaning of wells that have not been
<br />cleaned in nearly eighteen (18) years. She stated they encountered a lot of deterioration and
<br />buildup that needed more repair and replacement than anticipated. She noted this is critical work
<br />as use for water distribution. Mr. Gilot stated they encountered differing site conditions with
<br />unforeseen conditions. He noted the 20% limit on change orders is not expected for these kinds
<br />of situations. Ms. Rebecca Plantz, Engineering, added there was high iron build-up in this well
<br />with 3' of iron on the bottom. She noted it had to be cleaned. Upon a motion made by Ms.
<br />Dorau, seconded by Ms. Fritzberg and carried, the Change Order was approved.
