Employee is responsible for a portion <br />of the premium. Deductions will be made from first two payrolls of each month. Plan <br />uses Anthem Blue Cross and Blue Shield Network. <br />Deductibles, Physician Office Visit, Wellness, Hospital Care, Prescriptions <br />See attached Schedule of Benefits for detailed information. <br />Dental Insurance — PPO Network <br />Full time employees are eligible for dental insurance the first day of the month following <br />completion of 120 days of employment. Employee is responsible for the full premium <br />amount. Deductions will be made from each paycheck. <br />Type 1 Diagnostic & Preventative provides coverage up to 100 %. <br />Type II Basic Service covers up to 80% providing coverage for such services as fillings <br />and sealants. <br />Type III Major Services covers up to 50% with a 12 month waiting period for such <br />services as crowns and root canals. <br />Out -of- network coverage is the same with additional responsibility of costs exceeding <br />reasonable and customary. <br />Vision <br />Full time employees are eligible for vision insurance the first day of the month following <br />completion of 120 days of employment. Employee is responsible for the full premium <br />amount. Deductions will be..made from each paycheck. Coverage includes an eye <br />exam and lenses every 12 months and frames every 24 months after nominal co -pays. <br />Alternative $165.00 annual contact lens allowance permitted. Discount on Lasik <br />surgery. <br />401(k) <br />Full time employees are eligible to contribute to the 401(k) plan on the first entry up date <br />following completion of 6 months of employment.
