<br />APPROVE PROJECT COMPLETION AFFIDAVIT ---- COVELESKI STADIUM
<br />IMPROVEMENTS — MAIN FACILITY REHABILITATION — PROJECT NO. 111 -005A
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Ziolkowski Construction, Inc., 4050 Ralph Jones Drive, South Bend,
<br />Indiana 46628, for the above referenced project, indicating a final cost of $1,208,640.00. Upon a
<br />motion made by Mr. Inks, seconded by Ms. Roos and carried, the Project Completion Affidavit
<br />was approved.
<br />APPROVE CERTIFICATE OF SUBSTANTIAL COMPLETION — CLAY - CLEVELAND
<br />PUMP STATION REPLACEMENT — PROJECT NO. 109-087A (2010 SEWER BOND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Certificate of Substantial
<br />Completion on behalf of Selge Construction Company, Inc., 2833 S. 11`i' Street, Niles, Michigan
<br />49120, for the above referenced project, indicating a date of substantial completion of February
<br />2, 2012. Upon a motion made by Mr. Inks, seconded by Ms. Roos and carried, the Certificate of
<br />Substantial Completion was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS _ ONE (1) OR
<br />MORE, 2012 OR NEWER, FOUR (4) YARD FRONT -END RUBBER TIRE LOADERS —
<br />WASTEWATER CAPITAL
<br />In a memorandum to the Board, Mr. Jeff Hudak, Central Services, requested permission to
<br />advertise for the receipt of bids for the above referenced equipment. Therefore, upon a motion
<br />made by Mr. Inks, seconded by Ms. Roos and carried, the above request was approved.

<br />In a memorandum to the Board, Mr. Patrick. Henthorn, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Mr. Inks, seconded by Ms.
<br />Roos and carried, the above request to advertise was approved, and the Title Sheet was approved
<br />and signed.
