We can provide these services on request for
an additional fee once the results of the sampling under this proposal have identified the structures
involved.
COMPENSATION
For providing this Scope of Services, Grauvogel & Associates will be paid on an hourly rate basis, in
accordance with the following schedule, plus reimbursable expenses:
Classification
Hourly Rate
Certified Industrial Hygienist
$100.00
IDEM Asbestos Inspector/Planner
$100.00
Certified Hazardous Materials Manager
$100.00
Reimbursable expenses will consist of laboratory charges and sample shipping/handling charges.
Compensation under this Agreement will be allocated as follows:
LABOR — Per Structure FEE
Asbestos - Containing Materials (ACM) Survey $100.00
ACM Survey Report/IDEM Notification
100.00
Subtotal $200.00
REIMBURSABLES — Per Structure
ACM Analyses
PLM: 6 per structure @ $15.00 each $ 90.00
Basic Fee - Per Structure $290.00
BASIC FEE — All Structures (9) $2,610.00
