South Bend Redevelopment Commission <br />Ignition Park ACM Demolition Proposal — Revision 1 <br />4/7/2011 Page 2 <br />• complete the appropriate sections of the IDEM Notification of Demolition and Renovation <br />Operations; provide the Notification in printed and electronic format for the use by the <br />Commission and prospective demolition contractors <br />• identify as part of the survey other materials that will need special consideration for demolition; <br />this will include, but not be limited to fuel oil tanks (above and below ground) and RCRA <br />hazardous wastes (paint, oil, household wastes) <br />Suspect asbestos - containing material sampling will follow the protocol established by the EPA in the <br />AHERA regulations (40 CFR 763) for schools. Although these regulations do not have jurisdiction <br />over these buildings, they present a logical, comprehensive protocol for identification of ACM which <br />gives accurate and reproducible results. Our protocol will incorporate the following definitions: <br />• ALL: We will identify and classify all accessible materials. <br />• ACCESSIBLE: Only materials which are accessible without penetration behind existing walls, <br />floors and ceilings will be sampled; multiple layers on these surfaces will be sampled. <br />• ASSUMED: Suspect ACM enclosed behind existing walls, floors and ceilings will be assumed to <br />be the same material as those portions of the surface or similar surfaces which are accessible. <br />Our scope of services under this Agreement does not include assistance during the design, bid <br />procurement or construction phases of the project. We can provide these services on request for an <br />additional fee once the results of the sampling under this proposal have identified the structures <br />involved. Our normal scope for these additional services is as follows: <br />• prepare technical specifications for asbestos abatement, to be incorporated into the Contract <br />Documents for the project <br />• review the contractor's procedures and operations for compliance with the Contract Documents <br />and USEPA, IDEM and DOT regulations, to include: worker training, respiratory protection, <br />equipment, containment barriers, EPA/IDEM notifications and waste material manifests <br />• provide post- abatement inspection, for final release of each structure to the demolition contractor <br />• provide documentation of the asbestos abatement activities to the Owner for record purposes <br />COMPENSATION <br />For providing this Scope of Services, Grauvogel & Associates will be paid on an hourly rate basis, in <br />accordance with the following schedule, plus reimbursable expenses: <br />Classification <br />Hourl" `Raze <br />Certified Industrial Hygienist <br />$100.00 <br />IDEM Asbestos Inspector /Planner <br />$100.00 <br />Certified Hazardous Materials Manager <br />$100.00 <br />Reimbursable expenses will consist of laboratory charges and sample shipping/handling charges. <br />Compensation under this Agreement will be allocated as follows: <br />
