340 <br />REGULAR MEETING <br />DECEMBER 18,1978 <br />FILING OF REVISED SCHEDULE FOR FUNDING FOR CRIME VICTIM COUNSELOR <br />John Cimino, Director of the Criminal Justice Planning Bureau, <br />submitted to the Board a revised schedule of funding to R.E.A.L. <br />Services. He noted that the original request was for payment <br />in two installments, but the installments will now be paid quarterly <br />The total amount of the contract remains the same. Upon motion <br />made by Mr. McMahon, seconded by Mr. Brunner and carried, the <br />notification of `.Crime Victim Counselor payments was filed. <br />FINAL CHANGE ORDER APPROVED — AIRPORT INDUSTRIAL PARK, PHASE III <br />The Final Change Order in the Airport Industrial Park, Phase III <br />was submitted to the Board by John Leszczynski, Manager of the <br />Bureau of Public Construction. The change order reflects a <br />deduction of $10.75 to adjust the contract to as -built quantities. <br />The total contract price is $27,939.92. Upon motion made by Mr. <br />McMahon, seconded by Mr. Brunner and carried, the change order <br />was approved. <br />FINAL CHANGE ORDER APPROVED - AIRPORT INDUSTRIAL PARK, PHASE IV <br />DIVISION E <br />The Final Change Order in the Airport Industrial Park, Phase IV, <br />Division E, was submitted to the Board by John Leszczynski, <br />Manager of the Bureau of Public Construction. The change order <br />reflects an increase of $25,055.40 to cover unclassified excavation <br />and additional drainage control. The total contract price is <br />$227,614.59. Upon motion made by Mr. McMahon, seconded by Mr. <br />Brunner and carried, the change order was approved. <br />FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT APPROVED <br />WERWINSKI STREET IMPROVEMENTS <br />The Final Change Order in the Contract with Ziolkowski Construction <br />Company for the Werwinski Street Improvements was submitted to the <br />Board by John Leszczynski, Manager of the Bureau of Public <br />Construction. The Change Order increases the contract by <br />$5,618.13 and covers pavement replacement and adjusting the plans <br />to as -built quantities. Upon motion made by Mr. McMahon, seconded <br />by Mr. Brunner and carried, the change order was approved. <br />The Project Completion Affidavit for the project was also submitted. <br />The total contract price is $17,395.13. Upon motion made by Mr. <br />McMahon, seconded by Mr. Brunner and carried, the Project Completion <br />Affidavit was approved and the Waiver of Lien filed. <br />FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT APPROVED <br />BENDIX DRIVE IMPROVEMENTS <br />The Final Change Order in the contract with Rieth Riley Construction <br />Company for the Bendix DriveImp__rovements was submitted to the Board <br />by John Leszczynski, Manager of the Bureau of Public Construction. <br />The Change Order increases the contract by $4003.05 for the repaving <br />of the intersection of Bendix and Linden and to adjust plans to <br />as -built quantities. Upon motion made by Mr. McMahon, seconded <br />by Mr. Brunner and carried, the change order was approved. <br />The Project Completion Affidavit for the project was also submitted. <br />The total contract price.is $32,902.25. Upon motion made by Mr. <br />McMahon, seconded by Mr. Brunner and carried, the Project Completion <br />Affidavit, containing a maintenance agreement until December 18, <br />1981, was approved <br />1 <br />
