<br />Ms. Fritzberg and carried, Change Order No. 1(Final) and the Project Completion Affidavit
<br />were approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— MORRIS PERFORMING ARTS CENTER PHANTOM OF THE OPERA CHANDELIER
<br />SUPPORT — PROJECT NO. 117-034 (BROADWAY THEATER LEAGUE)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of A&D Contractors, LLC, 1449 Kimber Lane, Suite 103, Evansville, IN 47715 indicating
<br />the contract amount be decreased by $10,000 for a new contract sum, including this Change
<br />Order, of $258,044.74. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $258,044.74. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />Fritzberg and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved. Mr. Gilot noted no public funds would be used for this project.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— 2017 COLD MILLING — PROJECT NO 117-051 (LRSA)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of J.L. Milling, Inc., 15262 Industrial Drive, Schoolcraft, MI 49087 indicating the contract
<br />amount be decreased by $11,070.41 for a new contract sum, including this Change Order, of
<br />$51,129.59. Additionally submitted was the Project Completion Affidavit indicating this new
<br />final cost of $51,129.59. Upon a motion made by Ms. Maradik, seconded by Ms. Fritzberg and
<br />carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— 2017 DOWNSPOUT DISCONNECT PROGRAM — PROJECT NO. 116-114
<br />(WASTEWATER O&M)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Acorn Landscaping, LLC, 3680 W. Sample Street, South Bend, IN 46619 indicating
<br />the contract amount be decreased by $11,026.67 for a new contract sum, including this Change
<br />Order, of $38,968.33. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $38,968.33. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />Fritzberg and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE SHEET —
<br />COLFAX AVENUE TWO-WAY CONVERSION — PROJECT NO 115-053A (WWDA TIF)
<br />In a memorandum to the Board, Mr. Roger Nawrot, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. Fritzberg and carried, the above request to advertise was approved, and the Title Sheet was
<br />approved and signed.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS WASTEWATER
<br />TREATMENT PLANT BELT FILTER PRESS NO 1 — PROJECT NO 118 019 (UTILITIES
<br />WASTEWATER, OTHER EQUIPMENT)
<br />In a memorandum to the Board, Mr. Jacob Klosinski, Wastewater, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Therefore, upon a motion made
<br />by Ms. Maradik, seconded by Ms. Dorau and carried, the above request was approved.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — WATER/
<br />WASTEWATER LABORATORY EQUIPMENT (UTILITIES WASTEWATER CAPITAL)
<br />In a memorandum to the Board, Mr. Jacob Klosinski, Wastewater, requested permission to
<br />advertise for the receipt of bids for the above referenced equipment. Therefore, upon a motion
<br />made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the above request was approved.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS ONE (1) OR MORE
<br />2018 OR NEWER, ONE (1) TON FOUR-WHEEL DRIVE PICKUP TRUCK INCLUDING
<br />ANIMAL TRANSPORT MODULE — SPEC H (ANIMAL CONTROL)
<br />In a memorandum to the Board, Mr. Jeff Hudak, Central Services, requested permission to
<br />advertise for the receipt of bids for the above referenced vehicle Therefore, upon a motion made
<br />by Ms. Maradik, seconded by Ms. Fritzberg and carried, the above request was approved.
