3'71 REGULAR MEETING SEPTEMBER 19, 1983 APPROVE PROJECT COMPLETION AFFIDAVIT AND,ACCEPT IRREVOCABLE LETTER OF CREDIT #231 (ERSKINE MANOR SECTIONS & 3) Mr. Leszczynski advised that a Project Completion Affidavit was being submitted at this time by Mr. Joseph E. Hickey, .President of Erskine Manor Associates, Inc., 1270 Honan Drive, South Bend, Indiana, in the amount of $60,355.00 for the above referred to project. Accordingly, Irrevocable Letter of Credit #231 from the American National Bank & Trust Company, Main and Washington Streets, South Bend, Indiana, in the amount of $6,03.6.00 was presented to the Board for filing. Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, the Project Completion Affidavit was approved and the Irrevocable Letter of Credit #231 was filed. APPROVE CONSTRUCTION CONTRACT - MANHOLE RESTORATION PROJECT (SOUTH MAIN STREET) - PRESSURE CONCRETE CONSTRUCTION Mr. Leszczynski advised that in accordance with the bid awarded on August 29, 1983, to Pressure Concrete Construction Com any,.315 South Court Street, Florence, AL, in the amount of p$31,134.00 for the above referred to project, a contract in said amount was being submitted for Board approval. Upon a motion made by Mr. Leszczynski, seconded by Mr. Kernan and carried, the contract was approved and the appropriate insurance, Performance Bond and Labor and Materials Payment Bond were filed. APPROVE CHANGE ORDER NO. 1 - RETENTION BASIN & DITCH SYSTEM - RUM VILLAGE INDUSTRIAL PARK (H. DE WULF MECHANICAL CONTRACTOR) Mr. Leszczynski presented to the Board Change Order No..l for the above referred to project. The Change Order calls for the contract amount to be increased by $11,815.00, bringing the new contract sum including.this Change Order to $278,046.05. The Change Order indicates the increased cost is due to additional excavation. The storm drainage ditch west of the Conrail tracks has been extended 1875 feet northeasterly. Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, the above Change Order was approved. APPROVE CONTRACTOR AND HOMEOWNER AGREEMENTS - BUREAU OF HOUSING NEIGHBORHOOD PAINT -UP PROGRAM Ms. Janet S. Holston, Director, Bureau of Housing, presented to the Board for approval the following Neighborhood Paint - Up Program Agreements: CONTRACTOR AND HOMEOWNER AGREEMENTS 1. Homeowner: Silas/Dovella West Address: 812 West Thomas Street Contract Price: $170.00 Contractor: Aardvark Construction 2. Homeowner: Marguerite Thilman Address: 806 East Howard Street Contract Price: $440.00 Contractor: Aardvark Construction Company 3. Homeowner: Linda Bolger Address: 629 East Haney Contract Price: $500.00 Contractor: Charles Brown Maintenance, Inc.
