3'71 <br />REGULAR MEETING SEPTEMBER 19, 1983 <br />APPROVE PROJECT COMPLETION AFFIDAVIT AND,ACCEPT IRREVOCABLE <br />LETTER OF CREDIT #231 (ERSKINE MANOR SECTIONS & 3) <br />Mr. Leszczynski advised that a Project Completion Affidavit <br />was being submitted at this time by Mr. Joseph E. Hickey, <br />.President of Erskine Manor Associates, Inc., 1270 Honan <br />Drive, South Bend, Indiana, in the amount of $60,355.00 <br />for the above referred to project. Accordingly, Irrevocable <br />Letter of Credit #231 from the American National Bank & <br />Trust Company, Main and Washington Streets, South Bend, <br />Indiana, in the amount of $6,03.6.00 was presented to the <br />Board for filing. Upon a motion made by Mr. Leszczynski, <br />seconded by Mr. Hill and carried, the Project Completion <br />Affidavit was approved and the Irrevocable Letter of <br />Credit #231 was filed. <br />APPROVE CONSTRUCTION CONTRACT - MANHOLE RESTORATION PROJECT <br />(SOUTH MAIN STREET) - PRESSURE CONCRETE CONSTRUCTION <br />Mr. Leszczynski advised that in accordance with the bid <br />awarded on August 29, 1983, to Pressure Concrete Construction <br />Com any,.315 South Court Street, Florence, AL, in the amount <br />of p$31,134.00 for the above referred to project, a contract <br />in said amount was being submitted for Board approval. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. <br />Kernan and carried, the contract was approved and the <br />appropriate insurance, Performance Bond and Labor and <br />Materials Payment Bond were filed. <br />APPROVE CHANGE ORDER NO. 1 - RETENTION BASIN & DITCH <br />SYSTEM - RUM VILLAGE INDUSTRIAL PARK (H. DE WULF <br />MECHANICAL CONTRACTOR) <br />Mr. Leszczynski presented to the Board Change Order No..l <br />for the above referred to project. The Change Order calls <br />for the contract amount to be increased by $11,815.00, <br />bringing the new contract sum including.this Change Order <br />to $278,046.05. The Change Order indicates the increased <br />cost is due to additional excavation. The storm drainage <br />ditch west of the Conrail tracks has been extended 1875 <br />feet northeasterly. Upon a motion made by Mr. Leszczynski, <br />seconded by Mr. Hill and carried, the above Change Order <br />was approved. <br />APPROVE CONTRACTOR AND HOMEOWNER AGREEMENTS - BUREAU <br />OF HOUSING NEIGHBORHOOD PAINT -UP PROGRAM <br />Ms. Janet S. Holston, Director, Bureau of Housing, presented <br />to the Board for approval the following Neighborhood Paint - <br />Up Program Agreements: <br />CONTRACTOR AND HOMEOWNER AGREEMENTS <br />1. Homeowner: Silas/Dovella West <br />Address: 812 West Thomas Street <br />Contract Price: $170.00 <br />Contractor: Aardvark Construction <br />2. Homeowner: Marguerite Thilman <br />Address: 806 East Howard Street <br />Contract Price: $440.00 <br />Contractor: Aardvark Construction Company <br />3. Homeowner: Linda Bolger <br />Address: 629 East Haney <br />Contract Price: $500.00 <br />Contractor: Charles Brown Maintenance, Inc. <br />
