338 <br />REGULAR MEETING <br />AUGUST 20, 1984 <br />Approval was recommended subject to the maintenance of a ten foot <br />(10') clear and unobstructed lane for emergency purposes. It <br />was noted that the Petitioner will be responsible for the place- <br />ment and removal of the barricades for this event with the bar- <br />ricades to be delivered by the Bureau of Traffic and Lighting. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and <br />carried, the above recommendation was accepted and the request <br />approved. <br />APPROVE HANDICAPPED PARKING PERMITS <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and <br />carried, the following three (3) handicapped parking permit <br />applications were approved and referred to the Deputy Controller's <br />office for issuance of permits: <br />Gladys H. Nordin, 2205 S. Franklin, S.B. <br />William M. Smith, M.D., 2801 L.W.W., S.B. <br />Elmer C. Blondell, 717 E. Altgeld, S.B. <br />APPROVE TITLE SHEET - WASHINGTON TWO-WAY STREET PROJECT <br />Mr. Leszczynski advised that the Title Sheet for the above referred <br />to project was being presented at this time for execution. Upon a <br />motion made by Mr. Leszczynski, seconded by Mr. Vance and carried, <br />the above Title Sheet was -approved and signed. <br />APPROVE CONTRACTOR'S BOND <br />Mr. Melvin Humphrey, Engineering Department, recommended that the <br />Contractor's Bond for William 0. Houk be approved effective <br />August 20, 1984. Upon a motion made by Mr. Leszczynski, seconded <br />by Mr. Vance and carried, the above Contractor's Bond, as recom- <br />mended, was approved. <br />FILING OF STREET LIGHT OUTAGE REPORT <br />The report submitted to the Board indicated a total of sixty-six <br />(66) outages for the period July 19, 1984 to August 17, 1984. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and <br />carried, the report was filed. <br />FILING OF ENVIRONMENTAL CLEAN-UP OF LOTS REPORT <br />A report from the Department of Code Enforcement indicating a <br />total of one -hundred seventy-three (173) properties cleaned from <br />August 13, 1984 to August 17, 1984 was submitted. Upon a motion <br />made by Mr. Vance, seconded by Mr. Leszczynski and carried, the <br />report as submitted was filed. <br />FILING OF CERTIFICATES OF INSURANCE <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and <br />carried, the following Certificates of Insurance were accepted <br />and filed: <br />1.
