REGULAR MEETING SEPTEMBER 29, 1987 ZIOLKOWSKI CONSTRUCTION, INC. P.O. Box 1106 South Bend, Indiana 46624 Quotation was submitted by Dave Field, Vice -President and is in the amount of $31,000.00. Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and carried, the above referred to Quotations were referred to the Street Department for review and recommendation. APPROVE AGREEMENT FOR USE OF PUBLIC RIGHT-OF-WAY FOR FOOT RACES AND SIMILAR EVENTS - CROP WALK (OCTOBER 11, 1987) Mr. Leszczynski advised that pursuant to Resolution No. 9-1985, establishing policies and fees for foot races and similar events, adopted by the Board of Public Works on March 4, 1985, the United Religious Community, sponsors of the CROP Walk, have fulfilled resolution requirements and the appropriate Agreement is being submitted to the Board for approval. It was noted that on June 23, 1987, the Board of Public Works approved the date for this event subject to the sponsor meeting all Resolution requirements. Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and carried, the Agreement as submitted was approved and executed. APPROVE AGREEMENT COVERING CONSTRUCTION OF HIGHWAY -RAILWAY GRADE CROSSING (1500 SOUTH PRAIRIE AVENUE AT CONRAIL) Mr. Leszczynski advised that Mr. R. E. Wheeler, Area Engineer, Division of Local Assistance, Indiana Department of Highways, 100 North Senate Avenue, Indianapolis, Indiana, has submitted the above referred to Agreement which entails the construction of a rubber pad crossing surface at the above referred to location. Upon a motion made by Mr. Vance, seconded by Mrs. DeClercq and carried, the Agreement as submitted was approved and executed. APPROVE EXTENSION OF CONTRACT FOR CLEANING SERVICES - EAST SECTOR POLICE STATION Mr. Leszczynski advised that submitted to the Board for approval was the Extension of Contract for Cleaning Services at the East Sector Police Substation between the City and Mrs. Lucille Grueger, 826 North St. Louis Boulevard. Mr. Leszczynski noted that on June 16, 1987, the Board entered into a Contract with Mrs. Grueger to provide janitorial services for the East Sector Police Station for a period from June 15, 1987 through August 31, 1987. The extension submitted today indicates that the contract shall be extended through December 31, 1987. The rate of compensation and all duties shall remain the same as in the Agreement dated June 16, 1987. Upon a motion made by Mr. Vance, seconded by Mrs. DeClercq and carried, the Extension as submitted was approved and executed. APPROVE CHANGE ORDER NOS. 6-7-8-9 - SKYWALK FROM CENTURY CENTER TO MARRIOTT HOTEL PROJECT Mr. Leszczynski advised that Wright Construction Corporation, P.O. Box 1306, Elkhart, Indiana and Cole Associates, Inc., 2211 East Jefferson Boulevard, South Bend, have submitted Change Orders in the following amounts in regards to the above referred to project: No. 6 INCREASE $ 880.00 No. 7 INCREASE $4,969.00 No. 8 INCREASE $5,000.92 No. 9 INCREASE $8,326.56
