Therefore, Mr. Leszczynski made a motion that the bids be awarded as recommended subject to the filing of the appropriate affirmative action documents by each bidder as required. Ms. Humphreys seconded the motion and it carried. APPROVE CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - MICHIGAN STREET PHASE II PROJECT Mr. Leszczynski advised that Carl J. Reinke & Sons, P.O. Box 3095, South Bend, Indiana, has submitted Change Order No. 2 (Final) indicating that the Contract amount be increased by $961.50 for a new Contract sum including this Change Order in the amount of $708,300.05. Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were approved and the appropriate three-year Maintenance Bond was accepted for filing. APPROVE APPLICATIONS FOR BIDDER QUALIFICATION/CERTIFICATION Mr. Leszczynski advised that the Affirmative Action Review Board has advised that they have found that the companies listed below have met the requirements of the South Bend Affirmative Action Ordinance and they therefore recommended approval by the Board: 1. K.A. Steel Chemicals, Inc. 2. Turf Specialities Corporation 3. Ulrich Chemical, Inc. 4. Benefit Consultants, Inc. 5. Ziker Shannon Corporation 6. Rose Fuel Materials Upon a motion made by Mrs. DeClercq, seconded by Mr. Leszczynski and carried, the recommendation was accepted and the applications approved. REQUEST OF ST. JOE VALLEY WATERCOLOR SOCIETY FOR ONE-WAY STREET DESIGNATION IN CONJUNCTION WITH LEEPER PARK ART FAIR - JUNE In a letter to the Board, Ms. Jennifer S. Fox, Director, St. Joe Valley Watercolor Society, 758 Travers Circle, Mishawaka, Indiana, advised that the St. Joe Valley Watercolor Society has been granted permission by the Park Department to use Leeper Park for the Leeper Park Art Fair on June 18-19, 1988. Ms. Fox stated that the Society has also secured the appropriate insurance for the event and have requested the assistance of the Civil Defense Police to assure the safety of the fair participants. To ease traffic congestion on the days of the fair, the Society is requesting that Riverside Drive, as it is adjacent to Leeper Park, be made one-way west and that Park Lane be made one-way east. It is also requested that both streets be posted with no parking to be enforced by the Civil Defense. Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys and carried, the request was referred to the Bureau of Traffic and Lighting and the Police Department Traffic Division for review and recommendation. REQUEST OF NATIONAL ASSOCIATION OF LETTER CARRIERS TO CONDUCT INTERSECTION SOLICITATIONS FOR MDA - JULY 10 .1988 - REFERRED Ms. Karen 0. Locke, District Director, Muscular Dystrophy Association, Commerce Building, 103 West Wayne Street, Suite 312, South Bend, Indiana, on behalf of the National Association of Letter Carriers, South Bend Branch #330, requested permission to hold intersection solicitations for the Muscular Dystrophy Association on Sunday, July 10, 1988 from 11:00 a.m. to 4:00 p.m. at the following locations:
