Therefore, Mr. Leszczynski made a motion <br />that the bids be awarded as recommended subject to the filing of <br />the appropriate affirmative action documents by each bidder as <br />required. Ms. Humphreys seconded the motion and it carried. <br />APPROVE CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVIT - MICHIGAN STREET PHASE II PROJECT <br />Mr. Leszczynski advised that Carl J. Reinke & Sons, P.O. Box <br />3095, South Bend, Indiana, has submitted Change Order No. 2 <br />(Final) indicating that the Contract amount be increased by <br />$961.50 for a new Contract sum including this Change Order in the <br />amount of $708,300.05. Additionally submitted was the Project <br />Completion Affidavit indicating this new final cost. Upon a <br />motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and <br />carried, Change Order No. 2 (Final) and the Project Completion <br />Affidavit were approved and the appropriate three-year <br />Maintenance Bond was accepted for filing. <br />APPROVE APPLICATIONS FOR BIDDER QUALIFICATION/CERTIFICATION <br />Mr. Leszczynski advised that the Affirmative Action <br />Review Board <br />has advised that they have found that the companies <br />listed below <br />have met the requirements of the South Bend Affirmative <br />Action <br />Ordinance and they therefore recommended approval by <br />the Board: <br />1. K.A. Steel Chemicals, Inc. <br />2. Turf Specialities Corporation <br />3. Ulrich Chemical, Inc. <br />4. Benefit Consultants, Inc. <br />5. Ziker Shannon Corporation <br />6. Rose Fuel Materials <br />Upon a motion made by Mrs. DeClercq, seconded by Mr. <br />Leszczynski <br />and carried, the recommendation was accepted and the <br />applications <br />approved. <br />REQUEST OF ST. JOE VALLEY WATERCOLOR SOCIETY FOR ONE-WAY STREET <br />DESIGNATION IN CONJUNCTION WITH LEEPER PARK ART FAIR - JUNE <br />In a letter to the Board, Ms. Jennifer S. Fox, Director, St. Joe <br />Valley Watercolor Society, 758 Travers Circle, Mishawaka, <br />Indiana, advised that the St. Joe Valley Watercolor Society has <br />been granted permission by the Park Department to use Leeper Park <br />for the Leeper Park Art Fair on June 18-19, 1988. Ms. Fox stated <br />that the Society has also secured the appropriate insurance for <br />the event and have requested the assistance of the Civil Defense <br />Police to assure the safety of the fair participants. <br />To ease traffic congestion on the days of the fair, the Society <br />is requesting that Riverside Drive, as it is adjacent to Leeper <br />Park, be made one-way west and that Park Lane be made one-way <br />east. It is also requested that both streets be posted with no <br />parking to be enforced by the Civil Defense. <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys <br />and carried, the request was referred to the Bureau of Traffic <br />and Lighting and the Police Department Traffic Division for <br />review and recommendation. <br />REQUEST OF NATIONAL ASSOCIATION OF LETTER CARRIERS TO CONDUCT <br />INTERSECTION SOLICITATIONS FOR MDA - JULY 10 .1988 - REFERRED <br />Ms. Karen 0. Locke, District Director, Muscular Dystrophy <br />Association, Commerce Building, 103 West Wayne Street, Suite 312, <br />South Bend, Indiana, on behalf of the National Association of <br />Letter Carriers, South Bend Branch #330, requested permission to <br />hold intersection solicitations for the Muscular Dystrophy <br />Association on Sunday, July 10, 1988 from 11:00 a.m. to 4:00 p.m. <br />at the following locations: <br />
