M <br />REGULAR MEETING <br />APRIL 18, 1988 <br />and that work shall be completed within sixty (60) work days from <br />the date the Proposal is signed. <br />Upon a motion made by Mrs. DeClercq, seconded by Ms. Humphreys <br />and carried, the Proposal as submitted was approved and executed. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - CAST -IN PLACE CONCRETE <br />(STANLEY COVELESKI REGIONAL BASEBALL STADIUM PROJECT) <br />Mr. Leszczynski advised that Carl J. Reinke & Sons, Inc., has <br />submitted the Project Completion Affidavit for the Cast -In Place <br />Concrete Package for the Stanley Coveleski Regional Baseball <br />Stadium Project indicating a final cost of $1,162,575.52. Mr. <br />Leszczynski stated that these are not additional funds but merely <br />the closeout of the project. Also submitted with the Affidavit <br />was the appropriate three (3) year Maintenance Bond. Upon a <br />motion made by Ms. Humphreys, seconded by Mrs. DeClercq and <br />carried, the Project Completion Affidavit was approved and <br />executed and the Maintenance Bond was filed. <br />APPROVE PROJECT COMPLETION AFFIDAVIT. DEED OF DEDICATION. OWNER'S <br />CERTIFICATION AND EASEMENTS NO. 1 AND NO. 2 - SAINT JOSEPH <br />MEDICAL CENTER PROJECT <br />Mr. Leszczynski advised that Mr. Michael P. Meeks, Director, <br />Division of Engineering, submitted to the Board a Project <br />Completion Affidavit and three (3) year Maintenance Bond along <br />with an Easement that contains the public improvement on behalf <br />of the City of South Bend that was constructed for the Saint <br />Joseph Medical Center. Mr. Leszczynski explained that a sewer <br />had to be relocated for expansion at the Medical Center and <br />submitted today are the easements for the City's acceptance. <br />It was noted that the Project Completion Affidavit and <br />Maintenance Bond, submitted by James A. Peltz, Slutsky Peltz <br />Plumbing and Heating Company, Inc. indicates that the improvement <br />entails the abandonment of an old egg shaped brick sanitary sewer <br />in vacated Madison Street and construction of a new 30" cast iron <br />sewer in the new easement. <br />Easement No. 1 indicated that the described easements are for the <br />installation of and/or maintenance of existing underground <br />utilities and being located in the blocks bounded by Madison <br />Street, Notre Dame Avenue, St. Peter Street, St. Louis Boulevard <br />and the east/west alley north of LaSalle Avenue, between Notre <br />Dame Avenue and St. Peter Street, all located in Samuel <br />Cottrell's First Addition to Lowell, now a part of the City of <br />South Bend, Indiana. Easement No. 2 indicates that the easement <br />is for the purpose of installation of and/or maintenance of <br />existing underground utilities. <br />Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq <br />and carried, the Project Completion Affidavit, Deed of Dedica- <br />tion, Owner's Certification and Easements No. 1 and No. 2 were <br />accepted and approved. <br />APPROVE INDIANA DEPARTMENT OF HIGHWAYS EXTRA WORK AGREEMENTS NO. <br />1 AND NO. 2 - LOGAN STREET WIDENING PROJECT <br />Mr. Leszczynski advised that submitted to the Board were Indiana <br />Department of Highways Extra Work Agreements - Change Order Nos. <br />1 and 2. The Change Order amounts and description of extra work <br />are as follows: <br />CHANGE ORDER NO. 1 - $4,800.00 <br />This amount is for reimbursement for a grand trunk railroad <br />flagman needed when working inside railroad right-of-way. <br />
