Lai <br />REGULAR MEETING <br />JANUARY 25, 1988 <br />DEPARTMENT OF CODE ENFORCEMENT <br />DEMOLITION AND CLEARANCE ACTIVITY <br />The targeted goal for this activity is to demolish and clear <br />twelve (12) units within the Neighborhood Revitalization <br />Area. The total cost of this activity shall not exceed <br />$20,000.00. <br />LEGAL SERVICES CORPORATION OF NORTHERN INDIANA INC. <br />LEGAL SERVICES ACTIVITY <br />The targeted goal for this activity is to provide 900 <br />persons with legal counseling or referral services. The <br />total cost of this activity shall not exceed $38,500.00. <br />MADISON CENTER, INC. <br />RAPE CRISIS COUNSELING ACTIVITY <br />The targeted goal for this activity is to counsel forty <br />(40) victims and provide outreach and support services to <br />1,065 non -victim individuals. The total cost of this <br />activity shall not exceed $25,550.00. <br />COMMUNITY COORDINATED CHILD CARE, INC. <br />DAY CARE STIPEND ACTIVITY <br />The targeted goal for this activity is to provide 3,500 <br />child days of service. The total cost of this activity <br />shall not exceed $45,850.00. <br />CANCO, INC. <br />CANCO NEGLECT PREVENTION ACTIVITY <br />The targeted goal for this activity is to provide crisis <br />intervention counseling to 80 families within the Community <br />Development Neighborhood Revitalization Area. The total <br />cost of this activity shall not exceed $26,000.00. <br />Upon a motion made by Ms. Humphreys, seconded by Mr. Leszczynski <br />and carried, the above referred to Community Development <br />Contracts were approved and executed. <br />APPROVE CHANGE ORDERS NO. 1 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVITS - CONCRETE PAVEMENT REPAIRS PROJECT - PACKAGE A AND B <br />Mr. Leszczynski advised that LaPorte Construction Company, Inc., <br />P.O. Box 577, LaPorte, Indiana, has submitted Change Orders No. 1 <br />(Final) and Project Completion Affidavits for the following <br />projects: <br />CONCRETE PAVEMENT REPAIRS PROJECT - PACKAGE A AND B: <br />PACKAGE A: Decrease $3,672.50 <br />NEW CONTRACT AMOUNT INCLUDING C.O.: $43,775.00 <br />PACKAGE B: Decrease $7,851.17 <br />NEW CONTRACT AMOUNT INCLUDING C.O�: $34,778.33 <br />Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq <br />and carried, the above'Change Orders No. 1 (Final) and Project <br />Completion Affidavits indicating the new final costs were <br />approved and the appropriate three-year Maintenance Bonds as <br />submitted were accepted for filing. <br />APPROVE PROJECT COMPLETION AFFIDAVITS: <br />- INDIANA AVENUE/MASTIC CORPORATION PROJECT <br />- INSTALLATION OF TRAFFIC SIGNALS - SUGAR/MAPLE-BENDIX DRIVE <br />Mr. Leszczynski advised that Rieth-Riley Construction Company, <br />Inc., has submitted a Project Completion Affidavit for the <br />Indiana Avenue/Mastic Corporation Project indicating a final cost <br />of $166,339.93.
