Ut <br />REGULAR MEETING <br />JULY 1, 1991 <br />APPROVE CHANGE ORDER NO. 5 - REMODELING OF NEW EQUIPMENT SERVICES <br />BUILDING - 1045 WEST SAMPLE STREET <br />Mr. Leszczynski advised that The Troyer Group, Inc., 415 <br />Lincolnway East, Mishawaka, Indiana and L-.L. Geans, Inc., 56535 <br />Magnetic Drive, Mishawaka, Indiana, have submitted Change Order <br />No. 5 indicating that the Contract amount be increased by <br />$1,546.00 for a new Contract sum, including this Change Order in <br />the amount of $840,063.00. Ms. Humphreys therefore made a motion <br />that the Change Order No. 5 as submitted be approved and <br />executed. Mr. Leszczynski seconded the motion which carried. <br />APPROVE CHANGE ORDERS - INDIANA DEPARTMENT OF HIGHWAYS - DOWNTOWN <br />SIGNALIZATION PROJECT <br />In a letter to the Board, Mr. Joe J. Bellina, III, Project <br />Engineer, Assistant Manager Bureau of Public Construction, <br />submitted three (3) Change Orders necessary for additional <br />construction to complete the downtown signal project. Mr. <br />Bellina noted that the major emphasis of this work is the <br />installation of Walk - Don't Talk indications at intersections <br />which do not have this feature. Mr. Bellina further noted that <br />all this work is one hundred per cent (1000) federally funded. <br />Mr. Leszczynski noted that the Change Orders indicate increases <br />of $28,755.60, $2,520.00 and $6,660.00 and are for a variety of <br />items including loop wires and pedestal foundations. Ms. <br />Humphreys made a motion that the Change Orders as submitted be <br />approved. Mr. Leszczynski seconded the motion which carried. <br />In response to questions from the news media concerning this <br />project, Mr. Leszczynski advised that the work being done <br />presently on this project is the installation of conduits which <br />will accommodate the cabling. Mr. Leszczynski further advised <br />that it is anticipated that this project will be completed in <br />approximately October or November of this year. <br />APPROVE CHANGE ORDER NO. 2 (FINAL) - CONTRACT III - WASTEWATER <br />TREATMENT PLANT IMPROVEMENTS PROJECT <br />Mr. Leszczynski advised that submitted to the Board was Change <br />Order No. 2 (Final) for the above referred to project indicating <br />an increase of $19,114.97 for a new Contract amount of <br />$1,689,751.50. It is noted that Tenny Pavoni Associates, Inc., <br />and Superior Construction Company, Inc., have already executed <br />this Change Order. Ms. Humphreys made a motion that the Change <br />Order be approved. Mr. Leszczynski seconded the motion which <br />carried. <br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS FOR SIX (6) <br />MECHANICAL STREET SWEEPERS AND FOUR (4) TANDEM AXLE TRUCKS - <br />STREET DEPARTMENT <br />Mr. John E. Leszczynski, Director, Department of Public Works, <br />requested that the Board advertise for the receipt of bids for <br />six (6) mechanical street sweepers and four (4) tandem axle <br />trucks. Mr. Leszczynski noted that this equipment is to be used <br />in the South Bend Street Department and that funding has been <br />approved by the Controller's office and the South Bend Common <br />Council. Ms. Humphreys made a motion that the request be <br />approved. Mr. Leszczynski seconded the motion which carried. <br />REQUESTS REFERRED: <br />Mr. Leszczynski advised that the following requests have been <br />submitted to the Board: <br />
