Ut REGULAR MEETING JULY 1, 1991 APPROVE CHANGE ORDER NO. 5 - REMODELING OF NEW EQUIPMENT SERVICES BUILDING - 1045 WEST SAMPLE STREET Mr. Leszczynski advised that The Troyer Group, Inc., 415 Lincolnway East, Mishawaka, Indiana and L-.L. Geans, Inc., 56535 Magnetic Drive, Mishawaka, Indiana, have submitted Change Order No. 5 indicating that the Contract amount be increased by $1,546.00 for a new Contract sum, including this Change Order in the amount of $840,063.00. Ms. Humphreys therefore made a motion that the Change Order No. 5 as submitted be approved and executed. Mr. Leszczynski seconded the motion which carried. APPROVE CHANGE ORDERS - INDIANA DEPARTMENT OF HIGHWAYS - DOWNTOWN SIGNALIZATION PROJECT In a letter to the Board, Mr. Joe J. Bellina, III, Project Engineer, Assistant Manager Bureau of Public Construction, submitted three (3) Change Orders necessary for additional construction to complete the downtown signal project. Mr. Bellina noted that the major emphasis of this work is the installation of Walk - Don't Talk indications at intersections which do not have this feature. Mr. Bellina further noted that all this work is one hundred per cent (1000) federally funded. Mr. Leszczynski noted that the Change Orders indicate increases of $28,755.60, $2,520.00 and $6,660.00 and are for a variety of items including loop wires and pedestal foundations. Ms. Humphreys made a motion that the Change Orders as submitted be approved. Mr. Leszczynski seconded the motion which carried. In response to questions from the news media concerning this project, Mr. Leszczynski advised that the work being done presently on this project is the installation of conduits which will accommodate the cabling. Mr. Leszczynski further advised that it is anticipated that this project will be completed in approximately October or November of this year. APPROVE CHANGE ORDER NO. 2 (FINAL) - CONTRACT III - WASTEWATER TREATMENT PLANT IMPROVEMENTS PROJECT Mr. Leszczynski advised that submitted to the Board was Change Order No. 2 (Final) for the above referred to project indicating an increase of $19,114.97 for a new Contract amount of $1,689,751.50. It is noted that Tenny Pavoni Associates, Inc., and Superior Construction Company, Inc., have already executed this Change Order. Ms. Humphreys made a motion that the Change Order be approved. Mr. Leszczynski seconded the motion which carried. APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS FOR SIX (6) MECHANICAL STREET SWEEPERS AND FOUR (4) TANDEM AXLE TRUCKS - STREET DEPARTMENT Mr. John E. Leszczynski, Director, Department of Public Works, requested that the Board advertise for the receipt of bids for six (6) mechanical street sweepers and four (4) tandem axle trucks. Mr. Leszczynski noted that this equipment is to be used in the South Bend Street Department and that funding has been approved by the Controller's office and the South Bend Common Council. Ms. Humphreys made a motion that the request be approved. Mr. Leszczynski seconded the motion which carried.
