1,5 REGULAR MEETING MAY 13, 1991 ADDENDUM I NEIGHBORHOOD HOUSING SERVICES OF SOUTH BEND NHS REVOLVING LOAN FUND ACTIVITY Addendum I extends the Contract for this activity until December 31, 1993. ADDENDUM I NEIGHBORHOOD HOUSING SERVICES Addendum I extends the 1991 Contract to December 31, 1991. Upon a motion made by Ms. Humphreys, seconded by Mrs. DeClercq -and carried, the above referred to Contracts and Addenda were approved and executed. ACCEPT AND APPROVE STORM SEWER EASEMENTS - CORBY/TWYCKENHAM STORM SEWER PROJECT Mr. Leszczynski advised that in conjunction with the Corby/Twyckenham Storm Sewer project, submitted to the Board at this time are two (2) storm sewer Easements as provided to the City by Mr. John Friedline. It is noted that one (1) permanent Easement is thirty feet (301) in width and the other Temporary Construction Right -of -Way Easement is twenty feet (201) and parallel to the permanent easement. It is also noted that these Easements are a necessary part of the most crucial segment of the first phase of the Corby/Twyckenham Storm Sewer Project proposed for contract letting in August of 1991. Therefore, Mr. Leszczynski made a motion that these Easements be accepted and approved. Mrs. DeClercq seconded the motion which carried. APPROVE AT&T INDIVIDUAL COMMISSION AGREEMENT - PTI AMENDMENT Board Attorney Jenny Pitts Manier advised that submitted to the Board is an Amendment to the City's pay telephone Commission Agreement which indicates the amount of commission the City receives for all the pay telephones located in the City's public buildings. It was noted that the Agreement indicates a commission rate of fourteen per cent (14%) for nineteen (19) telephones. It is further noted that the AT&T Individual Commission Agreement - PTI Amendment, as submitted, is between AT&T Communications, Inc., acting on behalf of the Interstate Division of the American Telephone and Telegraph Company and the AT&T Communications interexchange companies and the City. Upon a motion made by Ms. Humphreys, seconded by Mrs. DeClercq and carried, the Agreement was approved and executed. APPROVE CONTRACT FOR CONSULTING SERVICES Mr. Leszczynski advised that submitted to the Board was a Contract for Consulting Services between Sam Mercantini and the City of South Bend by and through the Board of Public Works on behalf of the Department of Economic Development. The term of the Contract is January 1, 1991 through December 31, 1991. The Contract indicates that Mr. Mercantini shall be paid an amount not to exceed $24,720.00 for services provided under this Contract which include all aspects of the commercial revolving loan fund program including assistance to the Neighborhood Business Development Corporation Loan Committee. Mrs. DeClercq made a motion that this Contract be approved and executed. Ms. Humphreys seconded the motion which carried. 1
