1,5 <br />REGULAR MEETING <br />MAY 13, 1991 <br />ADDENDUM I <br />NEIGHBORHOOD HOUSING SERVICES OF SOUTH BEND <br />NHS REVOLVING LOAN FUND ACTIVITY <br />Addendum I extends the Contract for this activity until <br />December 31, 1993. <br />ADDENDUM I <br />NEIGHBORHOOD HOUSING SERVICES <br />Addendum I extends the 1991 Contract to December 31, 1991. <br />Upon a motion made by Ms. Humphreys, seconded by Mrs. DeClercq <br />-and carried, the above referred to Contracts and Addenda were <br />approved and executed. <br />ACCEPT AND APPROVE STORM SEWER EASEMENTS - CORBY/TWYCKENHAM STORM <br />SEWER PROJECT <br />Mr. Leszczynski advised that in conjunction with the <br />Corby/Twyckenham Storm Sewer project, submitted to the Board at <br />this time are two (2) storm sewer Easements as provided to the <br />City by Mr. John Friedline. <br />It is noted that one (1) permanent Easement is thirty feet (301) <br />in width and the other Temporary Construction Right -of -Way <br />Easement is twenty feet (201) and parallel to the permanent <br />easement. It is also noted that these Easements are a necessary <br />part of the most crucial segment of the first phase of the <br />Corby/Twyckenham Storm Sewer Project proposed for contract <br />letting in August of 1991. Therefore, Mr. Leszczynski made a <br />motion that these Easements be accepted and approved. Mrs. <br />DeClercq seconded the motion which carried. <br />APPROVE AT&T INDIVIDUAL COMMISSION AGREEMENT - PTI AMENDMENT <br />Board Attorney Jenny Pitts Manier advised that submitted to the <br />Board is an Amendment to the City's pay telephone Commission <br />Agreement which indicates the amount of commission the City <br />receives for all the pay telephones located in the City's public <br />buildings. It was noted that the Agreement indicates a <br />commission rate of fourteen per cent (14%) for nineteen (19) <br />telephones. <br />It is further noted that the AT&T Individual Commission Agreement <br />- PTI Amendment, as submitted, is between AT&T Communications, <br />Inc., acting on behalf of the Interstate Division of the American <br />Telephone and Telegraph Company and the AT&T Communications <br />interexchange companies and the City. <br />Upon a motion made by Ms. Humphreys, seconded by Mrs. DeClercq <br />and carried, the Agreement was approved and executed. <br />APPROVE CONTRACT FOR CONSULTING SERVICES <br />Mr. Leszczynski advised that submitted to the Board was a <br />Contract for Consulting Services between Sam Mercantini and the <br />City of South Bend by and through the Board of Public Works on <br />behalf of the Department of Economic Development. The term of <br />the Contract is January 1, 1991 through December 31, 1991. The <br />Contract indicates that Mr. Mercantini shall be paid an amount <br />not to exceed $24,720.00 for services provided under this <br />Contract which include all aspects of the commercial revolving <br />loan fund program including assistance to the Neighborhood <br />Business Development Corporation Loan Committee. Mrs. DeClercq <br />made a motion that this Contract be approved and executed. Ms. <br />Humphreys seconded the motion which carried. <br />1 <br />
