REGULAR MEETING DECEMBER 13, 1993 AWARD BID - TENANT SPACE IMPROVEMENT - PALAIS ROYALE Mr. Leszczynski noted that the Board is in receipt of a Memorandum from Mr. Carl P. Littrell, Director, Division of Engineering, advising that on December 6, 1993, the Board opened five (5) bids for the remodeling of tenant space in the Palais Royale Building. Mr. Littrell stated that he has reviewed the bids and found the amounts are correct as read at the meeting. The bids were for one lump sum for all the specified work. Mr. Littrell recommended that the Board award the low bid of Gibson -Lewis, Inc., 1001 West llth Street, Mishawaka, Indiana, in the amount of $46,900.00. Mr. Littrell noted that the Department of Community and Economic Development concurs with this recommendation. Mrs. Manier made a motion that this recommendation be accepted and the bid be awarded. Mr. Leszczynski seconded the motion which carried. APPROVE PROJECT COMPLETION AFFIDAVIT - SANITATION GARAGE EXPANSION PROJECT Mr. Leszczynski advised that Gibson -Lewis, Inc_, 1001 West 11th Street, Mishawaka, Indiana, has submitted a Project Completion Affidavit for the above referred to project indicating a final cost of $457,549.00. Upon a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the Project Completion Affidavit was approved and executed. REQUEST TO CHANGE STREET NAME FROM MAPLE ROAD TO BENDIX DRIVE - REFERRED In a Memorandum to the Board, Mr. Larry D. Spradlin, Sr., Division of Engineering, advised that the Department of Public Works petitions the Board to change the name of Maple Road to Bendix Drive. Mr. Spradlin noted that Maple Road is located between Cleveland Road extension and Portage Road, a distance of 1134 feet. The name change is logical inasmuch as Maple Road follows the alignment of Bendix Drive to Portage Road. An existing Maple Road (paper street only), located northwest of this particular section, does not align with this referenced Maple Road. At present, no properties or buildings are addressed off Maple Road. Mr. Spradlin submitted with the request a map of the referenced area, a printout of the current property owners abutting Maple Road and the signatures of those property owners consenting to the name change. Mr. Caldwell made a motion that this request be referred to the appropriate departments and bureaus for review and recommendation. Mrs. Manier seconded the motion which carried. REQUESTS REFERRED: Upon a motion made by Mr. Leszczynski, seconded by Mr. Caldwell and carried, the following requests were referred to the appropriate City departments and bureaus for review and recommendation: OF AMERICAN DIABETES ASSOCIATION TO CONDUCT WALKTOBER-FEST 94' - OCTOBER 2, 1994 In a letter to the Board, Ms. Donna M. Swope, Regional Director, American Diabetes Association, 115 South Lafayette Boulevard, Suite 407, South Bend, Indiana, requested permission to conduct Walktober-fest 94' beginning at 9:00 a.m., on Sunday, October 2, 1994. Ms. Swope advised that approimately six hundred (600) people will participate in the Walk utilizing the route which she submitted. Participants will walk only on the sidewalks. OF ST.
