165 <br />REGULAR MEETING <br />MAY 10, 1993 <br />APPROVE AGREEMENT - HOLY CROSS PARKVIEW HOSPITAL <br />Mr. Leszczynski indicated that the Board is in receipt of an <br />Agreement between the City and Holy Cross Parkview Hospital, 1915 <br />Lake Avenue, Plymouth, Indiana. <br />Chief Assistant City Attorney Jenny Pitts Manier advised that this <br />is an Agreement with Holy Cross Parkview Hospital whereby they <br />agree to reduce their normal charges by a certain percentage for <br />all City employees and their dependents who use their services. <br />Mr. Caldwell made a motion that this Agreement be approved and <br />executed. Mr. Leszczynski seconded the motion which carried. <br />APPROVE WASTEWATER DISCHARGE PERMIT <br />In a letter to the Board, Ms. Megan Wright, Pretreatment <br />Coordinator, Division of Environmental Services, submitted the <br />following Wastewater Discharge Permit and recommended approval: <br />1. Mechanics Laundry & Supply, Inc. <br />838 Prairie Street <br />South Bend, Indiana 46625 <br />Ms. Wright noted that this is a new Permit. <br />Therefore, Mr. Leszczynski made a motion that the Permit be <br />approved. Mr. Caldwell seconded the motion which carried. <br />AWARD BID - ONE (1) 3 1/2 TON CAB AND CHASSIS <br />In a letter to the Board, Mr. Matthew L. Chlebowski, Director, <br />Division of Equipment Services, recommended that the Board award <br />the bid of Shamrock Ford Truck Sales, 4707 Western Avenue, South <br />Bend, Indiana, in the amount of $36,135.38 as follows: <br />One (1) 1994 Ford LN 800 $35,935.38 <br />Optional sliding rear window $ 200.00 <br />TOTAL AWARD: $36,135.38 <br />It is noted that bids for this equipment were opened by the Board <br />on April 26, 1993. <br />Mr. Caldwell made a motion that this recommendation be accepted <br />and the bid be awarded. Mr. Leszczynski seconded the motion which <br />carried. <br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS• <br />- ONE (1) TANDEM VIBRATORY ROLLER <br />- ONE (1) VACUUM STREET SWEEPER WITH ATTACHMENTS <br />- FOUR (4) ASPHALT PATCH BOXES <br />Mr. Matthew L. Chlebowski, Director, Division of Equipment <br />Services, advised the Board that on behalf of the Street <br />Department, he requests that the Board advertise for the receipt <br />of bids for the above referred to equipment. Mr. Leszczynski made <br />a motion that the request be approved. Mr. Caldwell seconded the <br />motion which carried. <br />REQUESTS REFERRED: <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Caldwell <br />and carried, the following requests were referred to the <br />appropriate City departments and bureaus for review and <br />recommendation: <br />OF RILEY HIGH <br />Mr. Anthony B <br />High School, <br />SCHOOL TO CONDUCT PAR <br />yrd, Assistant Princip <br />405 East Ewing Avenue, <br />DE - OCTOBER 1, 1993 <br />1, James Whitcomb Riley <br />South Bend, Indiana, <br />
