066 <br />REGULAR MEETING <br />FEBRUARY 1, 1993 <br />1. REVISED TO: STOP SIGN (4 WAY STOPS) <br />LOCATION: Victory, Rockne & McArthur <br />REMARKS: Temporary 4-way Stop during the <br />Edison Road widening project. <br />Victory, Rockne and McArthur <br />will be a detour route. <br />Construction to start on <br />February 15, 1993 thru October, <br />1993. <br />APPROVE VISION SERVICE PLAN - INDIANA VISION SERVICES. INC. <br />Mr. Leszczynski advised that the Board is in receipt of a Vision <br />Service Plan as submitted by Indiana Vision Services, Inc., 494 <br />South Emerson Avenue, Suite G, Greenwood, Indiana. Board <br />Attorney and Member Mary H. Mueller stated that this is the <br />Contract for vision services for City employees which has been <br />negotiated and approved by the Legal Department. Therefore, Mrs. <br />Mueller made a motion that the Agreement be approved and <br />executed. Mr. Leszczynski seconded the motion which carried. <br />APPROVE CONSULTANT AGREEMENT - CORBY STREET LIFT STATION <br />REPLACEMENT AND FORCEMAIN EXTENSION <br />Mr. Leszczynski stated that the Board is in receipt of a <br />Consultant Agreement with Cole Associates, 2211 East Jefferson <br />Boulevard, South Bend, Indiana, for engineering services for <br />surveys, preparation of plans, specifications and complete <br />construction documents for a new wetwell/drywell station, a lower <br />lift station bypass, and a forcemain extension, all in <br />conformance with the recommendations contained in the report by <br />Cole entitled Sewer Systems/Corby Street Lift Station Analysis <br />dated October 1992. Mr. Leszczynski noted that this Agreement <br />shall not exceed $51,050.00. Therefore, Mr. Caldwell made a <br />motion that this Consultant Agreement be approved and executed. <br />Mrs. Mueller seconded the motion which carried. <br />APPROVE CONSULTANT AGREEMENT - CONSTRUCTION OF QUALITY DRIVE <br />Mr. Leszczynski informed members of the Board that Peirce & <br />Associates, P.O. Box 8160, South Bend, Indiana, has submitted to <br />the Board for consideration a Consultant Agreement in regards to <br />the construction of Quality Drive in the Toll Road Industrial <br />Park. The Agreement indicates that Peirce will provide survey, <br />design and bid and construction documents for the construction of <br />Quality Drive. It is noted that Peirce shall be compensated a <br />lump sum fee of five thousand dollars ($5,000.00) to provide the <br />survey, design and construction documents and also to provide a <br />geotechnical investigation at the actual cost charged by a <br />subcontractor plus five per cent (50) with the cost of this <br />geotechnical investigation not to exceed one thousand five <br />hundred dollars ($1,500.00). Mrs. Mueller made a motion that <br />this Agreement be approved and executed. Mr. Leszczynski <br />seconded the motion which carried. <br />APPROVE AMENDMENT TO SPECIFICATIONS FOR TAXICAB SAFETY DEVICES <br />Board Attorney and Member Mary H. Mueller reminded members of the <br />Board that on January 18, 1993, the Board adopted Resolution No. <br />6-1993 which adopted specifications relating to safety equipment <br />for taxicabs. <br />Pursuant to Section (3) of said specifications, the City was in <br />the process of securing the appropriate decals which would be <br />sold to taxicab companies and which stated "Drop Safe Equipped" <br />"Driver Has No Key" in red luminous letters at least two (2) <br />inches in height. In this process, it was discovered that <br />letters two (2) inches high were too large. Therefore, Mrs. <br />Mueller asked that the Board accept an amendment to this language <br />to read: <br />1 <br />1 <br />1 <br />
