Box 628 <br />South Bend, Indiana 46624 <br />INCREASE: $ 2,090.00 <br />NEW CONTRACT AMOUNT INCLUDING C.O.: $120,090.00 <br />Mrs. Manier made a motion that the Change Order and Project <br />Completion Affidavit for the Chippewa Avenue Project be approved. <br />Mr. Leszczynski seconded the motion which carried. Additionally, <br />Mrs. Manier made another motion that the Change Order and Project <br />Completion Affidavit for the Septage Receiving/Scum Handling <br />Project be approved. Mr. Leszczynski seconded the motion which <br />carried. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - BRIDGE PAINTING - <br />STUDEBAKER CORRIDOR REDEVELOPMENT AREA - CAPITAL IMPROVEMENT PLAN <br />PHASE IA, CONTRACT 4 <br />Mr. Leszczynski stated that Universal Painting Company, Inc., 1319 <br />South Lafayette Boulevard, P.O. Box 4141, South Bend, Indiana, has <br />submitted the Project Completion Affidavit, for the above referred <br />to project, indicating a final cost of $22,492.00.
