j 219 <br />REGULAR MEETING <br />APRIL 25, 1994 <br />Affidavit indicating this new final cost. Upon a motion made by <br />Mr. Leszczynski, seconded by Mrs. Manier and carried, Change Order <br />No. 1 (Final) and the Project Completion Affidavit were approved. <br />APPROVE CHANGE ORDER NO. 3 - CONTROL, INFORMATION AND <br />COMMUNICATION SYSTEM - WASTEWATER TREATMENT PLANT <br />Mr. Leszczynski stated that Koontz -Wagner Electric Company, Inc., <br />3801 Voorde Drive, South Bend, Indiana, has submitted Change Order <br />No. 3, for the above referred to project, indicating that the <br />Contract amount be increased by $36,174.12 for a new Contract sum, <br />including this Change Order, in the amount of $1,137,353.32. <br />Mr. Jack J. Dillon, Director, Division of Environmental Services, <br />informed the Board that this Change Order includes replacement of <br />eleven (11) air handling units, sixteen (16) heaters, and <br />protective relays at eleven (11) sites. It also includes the <br />change to a spread spectrum radio system to link site #26 to site <br />#5 to facilitate communication to the central computer. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Caldwell <br />and carried, the Change Order was approved. <br />FILING OF RATES - TCI OF MICHIANA, INC. <br />Mr. Leszczynski advised that TCI Great Lakes, Inc., 111 Pfingsten <br />Road, Suite 400, Deerfield, Illinois, on behalf of TCI of <br />Michiana, Inc., 2351 Miracle Lane, Mishawaka, Indiana, has filed <br />with the Board an explanation of their present basic tier and <br />equipment rates. The report includes the following: <br />FOR BASIC SERVICES TIER AND EQUIPMENT RATES: <br />Program Service Rate <br />1. Number of channels on basic service tier: 15 <br />2. Current rate for basic service tier: 9.89 <br />3. Maximum permitted per channel rate: 0.640 <br />4. Maximum permitted rate for basic service tier: 9.60 <br />Mr. Leszczynski made a motion that this report be accepted for <br />filing and referred to the City Attorney's office for review. <br />Mrs. Manier seconded the motion which carried. <br />REJECT QUOTATIONS - EAST BANK WALKWAY - HOWARD STREET APPROACH <br />IMPROVEMENT <br />Mr. Carl P. Littrell, Director, Division of Engineering, advised <br />that on February 21, 1994, the Board opened four (4) Quotations <br />for the above referred to project. Mr. Littrell noted that he has <br />reviewed the Quotations and found that the amounts are correct as <br />read at the Board meeting and responsive to the specifications. <br />Mr. Littrell further informed the Board that the South Bend Park <br />Department has found a sponsor (Sunburst) who is willing to <br />contract and provide these improvements. No public funds will be <br />necessary. Therefore, Mr. Littrell recommended that no award be <br />made for this project and all Quotations be rejected. <br />Mrs. Manier made a motion that the recommendation be accepted and <br />the Quotations be rejected. Mr. Leszczynski seconded the motion <br />which carried. <br />APPROVE REQUESTS TO ADVERTISE FOR THE RECEIPT OF BIDS: <br />- ONE (1) FULL SIZE CARGO VAN AND ONE (1.)
