<br />Therefore, upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the
<br />Agreement was approved and executed.
<br />APPROVE COMMUNITY DEVELOPMENT CONTRACTS
<br />The following Community Development Contracts were presented to the Board for approval:
<br />1995 R.E.W.A.R.D. Contract $95,000 Grant assistance to 63 low and
<br />moderate income households
<br />1995 Staff Support - HDC $47,000 Provide staff support to HDC
<br />Staff Support - CHC $25,000 Provide Staff Support
<br />Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the above referred to
<br />Contracts were approved and executed.
<br />APPROVE CHANGE ORDER - NEAR NORTHWEST PARTNERSHIP CENTER SERVICE
<br />AREA - PROJECT NO. 95-52
<br />Mr. Caldwell advised that Mr. Bob Allen, Public Construction Manger, has submitted Change Order
<br />No. 1, on behalf of Rieth Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana
<br />indicating that the Contract amount be increased $1,580.35 for a new Contract sum including this
<br />Change Order in the amount of $37,797.30. Upon a motion made by Mrs. Manier, seconded by Mr.
<br />Caldwell and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER - NEAR WEST SIDE PARTNERSHIP CENTER SERVICE AREA
<br />- PROJECT NO. 94-89
<br />Mr. Caldwell advised that Mr. Bob Allen, Public Construction Manger, has submitted Change Order
<br />No. 1, on behalf of Phend & Brown, Post Office Box 218, Milford, Indiana indicating that the
<br />Contract amount be increased $3,181.84 for a new Contract sum including this Change Order in the
<br />amount of $130,920.34. Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER MODIFICATION - TWYCKENHAM DRIVE
<br />Mr. Caldwell advised that Mr. Toy Villa, Project Engineer, has submitted Change Order No. 9, on
<br />behalf of HRP Construction, 5777 West Cleveland Road, South Bend, Indiana, indicating that the
<br />Contract amount be increased $63,143.53 for a new Contract sum including this Change Order in
<br />the amount of $1,439,616.80. Mr. Villa stated that Change Order Number 9, which was originally
<br />approved on February 27,1995, for an increase of $70,035.06, was incorrect. Upon a motion made
<br />by Mrs. Manier, seconded by Mr. Caldwell and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER - NEAR NORTHWEST PARTNERSHIP CENTER SERVICE
<br />AREA -PROJECT NO. 95-10
<br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manger, has submitted Change
<br />Order No. 1 on behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend,
<br />Indiana indicating that the Contract amount be increased $26,514.49 for a new Contract sum
<br />including this Change Order in the amount of $179,649.69. Upon a motion made by Mr. Caldwell,
<br />seconded by Mrs. Manier and carried, the Change Order was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT - SITE CLEARANCE - 202 SOUTH
<br />CHAPIN AND 239-241 SOUTH CHAPIN - DIVISION B - UNDERGROUND STORAGE TANK
<br />REMOVAL - PROJECT NO 95-23
<br />Mr. Caldwell advised that Mr. Ken Herceg, Ken Herceg & Associates, 224 East Jefferson, South
<br />Bend, Indiana, has submitted the Project Completion Affidavit on behalf of Warner & Sons, 29099
<br />U.S. 33, Elkhart, Indiana, for the above referred to project, indicating a final cost of $14,791.50.
<br />Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Project
<br />Completion Affidavit was approved.
