REGULAR MEETING AUGUST 14, 1995 <br />AWARD BID - ONE (1) AERIAL LIFT BOOM TRUCK <br />Mr. Matthew Chlebowski, Director, Division of Equipment Services, advised the Board that on <br />August 7, 1995, bids were received and opened for the above referred to equipment. After reviewing <br />those bids, Mr. Chlebowski recommends that the Board award the contract to Dueco, N4 W22610 <br />Bluemound Road, Waukesha, Wisconsin, in the amount of $48,055.00. Therefore, Mr. Leszczynski <br />made a motion that the recommendation be accepted and the bid be awarded as outlined above. Mrs. <br />Manier seconded the motion which carried. <br />APPROVE CONTRACT FOR STAFF SUPPORT - CHRISTMAS IN APRIL <br />Mr. Leszczynski advised that the Board is in receipt of a Contract for staff support between <br />Christmas in April of St. Joseph County and the Department of Community and Economic <br />Development. It is noted that services to be provided include staff support. Therefore, upon a <br />motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Contract was approved and <br />executed. <br />APPROVAL OF AGREEMENT - SHIVELY FAMILY TRUST AND AT OF INDIANA. LLC <br />AND A RELATED ESCROW AGREEMENT <br />Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend <br />and the Shively Family Trust, for the extension of William Richardson Drive in the Toll Road <br />Industrial Park to provide access to a development site for AT of Indiana, LLC. The land owner, <br />the Shively Family Trust ,will provide half of the cost of construction of the road over their property, <br />or $40,000.00. The City of South Bend is committing to provide the funds for the cost of extending <br />the road to the Shively property. The Escrow Agreement establishes St. Joseph Title Corporation <br />as an agent to hold the Shively's commitment of $40,000.00 in escrow for the City to use during <br />construction of the road. Therefore, upon a motion made by Mrs. Manier, seconded by Mr. <br />Leszczynski and carried, the Agreement was approved and executed. <br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - <br />BLACKTHORN GOLF CLUB ADDITIONS TO CART PATHS - PROJECT NO. 95-47 <br />Mr. Leszczynski advised that Mr. Bob Allen, Manager of Public Construction, has submitted Change <br />Order No. 1 (Final), on behalf of Asphalt Paving Company, Inc., 1205 East 8th Street, Mishawaka, <br />Indiana, indicating that the contract amount be increased by $135.37 for a new contract sum <br />including this Change Order in the amount of $61,642.02. Additionally submitted was the Project <br />Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Leszczynski, <br />seconded by Mrs. Manier and carried, Change Order No. 1 (Final) and the Project Completion <br />Affidavit were approved subject to the filing of the appropriate three-year Maintenance Bond. <br />APPROVAL OF RECOMMENDATION - TO CONDUCT BLOCK PARTY <br />Mr. Leszczynski stated that favorable recommendations have been received by the Division of <br />Engineering, Police Department and the City Attorney's office, in reference to a request as submitted <br />by Mr. Paul Hayden, Historic District Yard Sale Committee, 914 Lincolnway West, South Bend, <br />Indiana to conduct the above referred to block parry, on Saturday, September 9,1995, from 8:00 a.m. <br />until 5:00 p.m., on LaPorte Avenue, from West Washington to West Colfax Streets, in conjunction <br />with the Historic District Yard Sale. Upon a motion made by Mr. Leszczynski, seconded by Mr. <br />Caldwell and carried, the recommendation was approved. <br />APPROVAL OF RECOMMENDATION - TO CONDUCT BLOCK PARTY <br />Mr. Leszczynski stated that favorable recommendations have been received by the Division of <br />Engineering, Police Department and the City Attorney's office, in reference to a request as submitted <br />by Mr. Robert Daily, 1203 East Wayne, South Bend, Indiana to conduct the above referred to block <br />party, on Saturday, August 26, 1995, from 10:00 a.m. until 12:00 p.m., on East Wayne Street South <br />from Eddy Street to Sunnyside. Upon a motion made by Mr. Leszczynski, seconded by Mr. <br />Caldwell and carried, the recommendation was approved. <br />
