REGULAR MEETING Zone Change Order 1 1 2 1 3 1 9 1 10 1 Amount $11,705.45 Decrease $ 688.28 Decrease $ 5,930.07 Increase $ 8,832.68 Decrease $51,504.44 Decrease FEBRUARY 12. 1996 Final Contract Sum $ 77,804.55 $ 88,821.72 $ 95,440.07 $ 80,677.32 $ 38,005.56 Additionally submitted were the Project Completion Affidavits indicating the new final costs. The appropriate three-year Maintenance Bonds were also submitted. Upon a motion made by Mr. Leszczynski, seconded by Mr. Caldwell and carried, Change Orders and the Project Completion Affidavits were approved. APPROVE LICENSE APPLICATIONS - PUBLIC PARKING FACILITY Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the following license applications were approved: The People's Church of South Bend 310 West Washington South Bend, Indiana E.H. McNease 418 South Main South Bend, Indiana E.H. McNease 420 South Main Street South Bend, Indiana The above license applications were approved and referred to the Deputy Controller's Office for issuance of licenses. It was noted that the required Proof of Insurance as well as the hours and rates to be charged were attached to the applications. APPROVAL OF LICENSE APPLICATIONS - RUBBISH AND GARBAGE REMOVAL SERVICE Mr. Leszczynski stated that the Board is in receipt of favorable recommendations from the Department of Code Enforcement for License Applications for Rubbish and Garbage Removal Service for the following applicants: Best Way Disposal Eddie Ivory 3290 Hennesey Road 1302 West Linden Watervliet, Michigan South Bend, Indiana Mr. Leszczynski stated that the Bureau of Sanitation has inspected the vehicles to be used in conjunction with the businesses. Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the License Applications were approved. APPROVAL OF RECOMMENDATION TO ENCROACH ONTO PUBLIC RIGHT-OF-WAY - VIDEOTAPING ON SIDEWALK AT COLFAX AVENUE AND BELLEVUE SOUTH OF MISHAWAKA AVENUE Mr. Leszczynski advised that the Board is in receipt of favorable recommendations from the Division of Engineering, Police Department, Building Commissioner and the Redevelopment Design Review Committee for a request as submitted by Mr. Sean A. Hoade, The Double-Crossers, 18115 State Road 23, South Bend, Indiana, for a request to videotape on the sidewalk of Colfax Avenue in the downtown area one morning between March 3 and March 10,1996, and on Bellevue just south of Mishawaka Avenue on a different day between the two dates. Further, it is to be noted that Mr. Hoade will be videotaping for a movie project under the auspices of Indiana University at South Bend. Upon a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the recommendation was approved.
