<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />SOUTHEAST REDEVELOPMENT AREA SITE CLEARANCE, PHASE I - PROJECT NO 94-72
<br />Mr. Leszczynski advised that Mr. Bob Allen, Project Manager has submitted Change Order No. 1
<br />(Final), on behalf of B&J Excavation,1104 West Donald Street, South Bend, Indiana, indicating
<br />that the contract amount be increased by $10,400.00 for a new contract sum including this Change
<br />Order in the amount of $100,375.00, Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost. Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and
<br />carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />GOOD NEIGHBORS PUBLIC WORKS PROGRAM - ZONE 11D - PROJECT NO. 95-20
<br />Mr. Leszczynski advised that Mr. Bob Allen, Contract Engineer has submitted Change Order No.
<br />1 (Final) on behalf of L.L. Geans, Inc., 1923 North Home Street, Mishawaka, Indiana, indicating that
<br />the contract amount be decreased by $22,555.63 for a new contract sum including this Change Order
<br />in the amount of $73,939.37. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost. Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and
<br />carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved subject to
<br />the filing of the appropriate three-year Maintenance Bond.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />GOOD NEIGHBORS PUBLIC WORKS PROGRAM - ZONE I IA - PROJECT NO. 95-20
<br />Mr. Leszczynski advised that Mr. Bob Allen, Contract Engineer has submitted Change Order No.
<br />1 (Final) indicating that the contract amount be decreased by $49,647.34 for a new contract sum
<br />including this Change Order in the amount of $46,847.66. Additionally submitted was the Project
<br />Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Caldwell,
<br />seconded by Mrs. Manier and carried, Change Order No. 1 (Final) and the Project Completion
<br />Affidavit were approved subject to the filing of the appropriate three-year Maintenance Bond.
<br />APPROVE CHANGE ORDER NO. 17 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />COLLEGE FOOTBALL HALL OF FAME - PROJECT NO. 93-09
<br />Mr. Leszczynski advised that Mr. Rick Dieringer, Verkler Inc., 4406 Technology Drive, South Bend,
<br />Indiana has submitted Change Order No. 00017 (Final) on behalf of Colip Electric, Post Office Box
<br />840, South Bend, Indiana, indicating that the contract amount be increased by $1,573.18 for a new
<br />contract sum including this Change Order in the amount of $656,323.15. Additionally submitted
<br />was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr.
<br />Caldwell, seconded by Mrs. Manier and carried, Change Order No. 17 (Final) and the Project
<br />Completion Affidavit were approved subject to the filing of the appropriate three-year Maintenance
<br />Bond.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />COLLEGE FOOTBALL HALL OF FAME - ATTA INC. - PROJECT NO 93-09
<br />Mr. Leszczynski advised that Mr. Bob Allen, Construction Engineer has submitted Change Order
<br />No. 1 (Final) on behalf of Atta Inc., 132 West 21 st Street, New York, New York, indicating that the
<br />contract amount be decreased by $6,400.00 for a new contract sum including this Change Order in
<br />the amount of $259,550.00. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost. Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried,
<br />Change Order No.1 (Final) and the Project Completion Affidavit were approved subject to the filing
<br />of the appropriate three-year Maintenance Bond.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />1995 SEWER LINING PROGRAM - PROJECT NO. 95-66
<br />Mr. Leszczynski advised that Mr. Toy Villa, Contract Engineer has submitted Change Order No. 1
<br />(Final) on behalf of Insituform Midwest, Inc., Post Office Box 458, Lemont, Illinois, indicating that
<br />the contract amount be increased by $6,900.00 for a new contract sum including this Change Order
<br />in the amount of $358,601.77. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost. Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and
<br />carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved subject to
<br />the filing of the appropriate three-year Maintenance Bond.
