( 9`' A_ <br />REGULAR MEETING MAY 26,108 <br />Upon a motion .made by Ms. Roemer, seconded by Mr. Powell and carried, the above bids were <br />referred to the Division of Engineering for review and recommendation. <br />OPENING OF QUOTATIONS - MAYFLOWER ROAD - PHASE IIA - PROJECT NO. 98-13 <br />Mr. Caldwell advised that this was the date set for the receiving and opening of sealed Quotations <br />for the above referred to project. The following Quotations were opened and read: <br />LAFREE EARTH INC. <br />Post Office Box 1752 <br />Mishawaka, Indiana 46546 <br />Quotation was submitted by Mr. Greg LaFree, President <br />QUOTATION: $ 68,342.00 <br />Alternate No. 1 $458,495.00 <br />YOUNGS EXCAVATING, INC. <br />50563 Dusty Trail Court <br />Granger, Indiana 46530 <br />Quotation was submitted by Youngs Excavating <br />QUOTATION: $ 70,323.35 <br />HERRMAN & GOETZ, INC. D/B/A/ UNDERGROUND SERVICES <br />225 South Lafayette <br />South Bend, Indiana 46601 <br />Quotation was submitted by Underground Services <br />QUOTATION: $ 114,983.82 <br />Quotations from Small, Inc., and HRP Construction were rejected, as they were not received before <br />the 9:30 a.m. meeting deadline. Small, Inc., and IMP Construction stated that an Addendum from <br />Cole Associates stipulated the deadline was 10:00 a.m. <br />Upon a motion made by Mr. Caldwell, seconded by Ms. Roemer and carried, the above Quotations <br />were referred to Division of Engineering for review and recommendation. <br />AWARD BID - ONE (1) 1,200 MINI TANK <br />Mr. Matthew Chlebowski, Director, Division of Equipment Services, advised the Board that on May <br />18, 1998, bids were received and opened for the above referred to tank. After reviewing those bids, <br />Mr. Chlebowski recommends that the Board award the contract to Stepp Manufacturing Company, <br />Inc. 12325 River Road, North Branch, Minnesota in the amount of $23,830.00. Therefore, Ms. <br />Roemer made a motion that the recommendation be accepted and the bid be awarded as outlined <br />above. Mr. Powell seconded the motion which carried. <br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - <br />WESTSIDE PARTNERSHIP CENTER SIDEWALK. CURB AND APPROACH REPAIRS - <br />PROJECT NO. 97-38 <br />Mr. Caldwell advised that Mr. Toy Villa, Construction Manager has submitted Change Order No. <br />1 (Final) on behalf of Batteast Construction Company, 430 East LaSalle, South Bend, Indiana, <br />indicating that the contract amount be decreased by $2.53 for a new contract sum including this <br />Change Order in the amount of $45,027.53. Additionally submitted was the Project Completion <br />Affidavit indicating this new final cost. Upon a motion made by Mr. Caldwell, seconded by Ms. <br />Roemer and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved <br />subject to the filing of the appropriate three-year Maintenance Bond. <br />it <br />1 <br />
