0-4 A <br />REGULAR MEETING <br />APRIL 19, 1999 <br />General Steel Fabricating Company, Inc., 5230 East 26 and L Street, Indianapolis, Indiana, for the <br />above referred to project. Mr. Caldwell noted that the Agreement is in the amount of $719,700.00. <br />Therefore, upon a motion made by Ms. Roemer, seconded by Mr. Caldwell and carried, the <br />Agreement was approved and executed. <br />APPROVAL OF AGREEMENT - BUILDING BLOCK GRANT PROGRAM <br />Mr. Caldwell stated that the Board is in receipt of an Agreement between the City of South Bend and <br />the Woodward Improvement Group, 1253 Woodward, South Bend, Indiana, for the costs associated <br />with planting sugar maples at 1260, 1252, 1246, 1249, 1245 and 1236 Woodward Avenue, and 1105 <br />Hudson Avenue. Mr. Caldwell noted that the Agreement is in the amount of $1,000.00. Therefore, <br />upon a motion made by Mr. Caldwell, seconded by Ms. Roemer and carried, the Agreement was <br />approved and executed. <br />APPROVAL OF PROPOSAL - PROFESSIONAL SERVICES - TOLL ROAD INDUSTRIAL <br />PARK <br />Mr. Caldwell stated that the Board is in receipt of a Proposal between the City of South Bend and <br />�j The Troyer Group, 550 South Union, Mishawaka, Indiana, for a revised drainage plan for the Toll <br />Road Industrial Park. Mr. Caldwell noted that the Proposal is in the amount of $2,925.00, and <br />funding has been appropriated through the Blackthorn Economic Development Tax Increment Fund. <br />Therefore, upon a motion made by Ms. Roemer, seconded by Mr. Powell and carried, the Proposal <br />was approved and executed. <br />APPROVE COMMUNITY DEVELOPMENT CONTRACT <br />The following Community Development Contract was presented to the Board for approval: <br />i Community Development Program Contract $70,000.00 Cover NNN Administrative costs <br />- Near Northwest Neighborhoods; Inc. <br />Upon a motion made by Ms. Roemer, seconded by Mr. Powell and carried, the above referred to <br />Contract was approved and executed. <br />APPROVAL OF PROPOSAL - ENGINEERING SERVICES - CONTAMINATED SOIL AND <br />GROUNDWATER CLEANUP - TRIAD ENGINEERING <br />J' Mr. Caldwell stated that the Board is in receipt of a Proposal between the City of South Bend and <br />Triad Engieering, 300 North Michigan, South Bend, Indiana, for engineering services related to the <br />removal of contaminated soil at the Chemical Bulding at the Wasteewater Treatment Plant. Mr. <br />Caldwell noted that the Proposal is in the amount of $25,000.00, and funding is budgeting in the <br />Wastewater operational budget. Therefore, upon a motion made by Ms. Roemer, seconded by Mr. <br />Powell and carried, the Proposal was approved and executed. <br />APPROVAL OF ADDENDUM TO AGREEMENT - PLANNING SERVICES - DOWNTOWN <br />SOUTH BEND STRATEGIC AND OPERATING PLAN - BUSINESS DISTRICTS. INC. <br />Mr. Caldwell stated that the Board is in receipt of an Agreement between the City of South Bend and <br />Bosiness Districts, Inc., Evanston, Illinois, for additional profeesional services for the above referred <br />to project. Mr. Caldwell noted that -the Agreement is in the amount of $4,000.00. Therefore, upon <br />a motion made by Mr. Caldwell, seconded by Ms. Roemer and carried, the Amendment to the <br />Agreement was approved and executed. <br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - <br />HOMELAND SUBDIVISION SEWER EXTENSION - PROJECT NO. 98-69 <br />Mr. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final <br />Change Order on behalf of Bradberry Brothers, 20061 Dice Street, South Bend, Indiana, indicating <br />that the contract amount be decreased by $2,625.00 for a new contract sum including this Change <br />Order in the amount of $14,885.00. Additionally submitted was the Project Completion Affidavit <br />indicating this new final cost. Upon a motion made by Ms. Roemer, seconded by Mr. Powell and <br />carried, the Final Change Order and the Project Completion Affidavit were approved subject to the <br />filing of the appropriate three-year Maintenance Bond. <br />1 <br />E <br />
