REGULAR MEETING JANUARY 11, 1999 <br />APPROVE CHANGE ORDER - MORRIS PERFORMING ARTS CENTER - EXCAVATION <br />AND SHEETING d PROJECT NO. 98-25-3A <br />Mr. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order <br />No. 001, on behalf of Ritschard Brothers, Inc., 1204 West Sample Street, South Bend, Indiana <br />indicating that the Contract amount be increased $16,939.00 for a new Contract sum including this <br />Change Order in the amount of $251,184.00. Upon a motion made by Ms. Roemer, seconded by Mr. <br />Caldwell and carried, the Change Order was approved. <br />APPROVE CHANGE ORDER - PROSPECT STREET IMPROVEMENTS - PROJECT NO. 98-70 <br />W. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order <br />No. 1 on behalf of R&R Excavating, 2010 Went Avenue, Mishawaka, Indiana indicating that the <br />Contract amount be increased $2,232.00 for a new Contract sum including this Change Order in the <br />amount of $285,650.25. Upon a motion made by Ms. Roemer, seconded by Mr. Caldwell and <br />carried, the Change Order was approved. <br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - <br />INTERSECTION IMPROVEMENTS - AMERITECH DRIVEXLEVELAND ROAD - PROJECT <br />NO. 97-98 <br />Mr. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final <br />Change Order on behalf of Rieth Riley Construction, 25200 State Road 23, South Bend, Indiana, <br />indicating that the contract amount be decreased by $13,342.58 for a new contract sum including this <br />Change Order in the amount of $248,017.05. Additionally submitted was the Project Completion <br />Affidavit indicating this new final cost. Upon a motion made by Mr. Caldwell, seconded by Ms. <br />Roemer and carried, the Final Change Order and the Project Completion Affidavit were approved <br />subject to the filing of the appropriate three-year Maintenance Bond. <br />APPROVE CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - <br />STATE THEATER REHABILITATION - PROJECT NO. 97-07-1 <br />Mr. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order <br />No. 2 (Final) on behalf of Ramenda Masonry, 54889 Lexington Avenue, South Bend, Indiana, <br />indicating that the contract amount be decreased by $5,600.00 for a new contract sum including this <br />Change Order in the amount of $227,451.60. Additionally submitted was the Project Completion <br />Affidavit indicating this new final cost. Upon a motion made by Mr. Powell, seconded by Mr. <br />Caldwell and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were <br />approved subject to the filing of the appropriate three-year Maintenance Bond. <br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - 1998 <br />PUBLIC WORKS PARTNERSHIP PROGRAM - ZONE 4 - PROJECT NO 98-27 <br />Mr. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final <br />Change Order on behalf of Walsh and Kelly, 24358 State Road 23, South Bend, Indiana, indicating <br />that the contract amount be decreased by $26,457.80 for a new contract sum including this Change <br />Order in the amount of $89,980.20. Additionally submitted was the Project Completion Affidavit <br />indicating this new final cost. Upon a motion made by Ms. Roemer, seconded by Mr. Powell and <br />carried, the Final Change Order and the Project Completion Affidavit were approved subject to the <br />filing of the appropriate three-year Maintenance Bond. <br />APPROVE CONSTRUCTION CONTRACT - HOMELAND SUBDIVISION SEWER <br />EXTENSION - PROJECT NO. 98-69 <br />Mr. Caldwell advised that in accordance with the bid awarded on December 21, 1998, to Bradberry <br />Brothers, 20061 West Dice, South Bend, Indiana, in the amount of $17,510.00 for the above referred <br />to project, a Contract in said amount was being submitted for Board approval. Upon a motion made <br />by Mr. Powell, seconded by Ms. Roemer and carried, the Contract was approved and the appropriate <br />Certificate of Insurance was filed. <br />APPROVE CONSTRUCTION CONTRACT - 1998 SEWER LINING PROGRAM - PROJECT <br />NO. 98-94 <br />Mr. Caldwell advised that in accordance with the bid awarded on December 21, 1998, to Insituform <br />Midwest, Inc., 12897 Main Street, Lemont, Illinois, in the amount of $767,701.60 for the above <br />referred to project, a Contract in said amount was being submitted for Board approval.
