<br />APPROVE FINAL CHANGE ORDER - ENTERPRISE DRIVE EXTENSION - LONE
<br />WOLF/EAGLE WAY - SMALL, INC. - PROJECT NO. 100-16
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manger, has submitted the Final Change
<br />Order on behalf of Small, Inc., 25190 State Road 4, North Liberty, Indiana, indicating that the
<br />Contract amount be increased $63,811.85 for a new Contract sum including this Change Order in the
<br />amount of $471,207.77. Upon a motion made by Mr. Inks, seconded by Mr. Gilot and carried, the
<br />Final Change Order was approved.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - LAUREL
<br />ROAD WATER AND SEWER EXTENSION - PROJECT NO 98-71
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manger has submitted the Final Change
<br />Order on behalf of Oselka Construction, Post Office Box 253, Union Pier, Michigan, indicating that
<br />the contract amount be decreased by $2,094.44 for a new contract sum including this Change Order
<br />in the amount of $407,512.56. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost. Upon a motion made by Mr. Inks, seconded by Mr. Gilot and carried,
<br />the Final Change Order and the Project Completion Affidavit were approved subject to the filing of
<br />the appropriate three-year Maintenance Bond. It was also noted that the Board of Public Works has
<br />a conditional Waiver of Lien for the project on file.
<br />APPROVE CHANGE ORDERS AND PROJECT COMPLETION AFFIDAVIT - BLACKTHORN
<br />COURT/CRESCENT CIRCLE - LANDSCAPING AND IRRIGATION - PROJECT NO. 100-30
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager has submitted Change Order No.
<br />1 on behalf of Willson Nursery and Landscaping, Post Office Box 266, LaPorte, Indiana, indicating
<br />that the contract amount be increased by $6,990.00 for a new contract sum including this Change
<br />Order in the amount of $105,707.00. Also submitted was the Final Change Order, for a decrease of
<br />$1,879.20, for a final contract amount of $103,827.80. Additionally submitted was the Project
<br />Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Gilot, seconded by
<br />Mr. Inks and carried, the Change Orders and the Project Completion Affidavit were approved subject
<br />to the filing of the appropriate three-year Maintenance Bond.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT -
<br />SOUTHEAST NEIGHBORHOOD IMPROVEMENTS - PROJECT NO 100-91
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager has submitted the Final Change
<br />Order, with no monetary change, for a new contract sum including this Change Order in the amount
<br />of $69,850.00. Additionally submitted was the Project Completion Affidavit indicating this new final
<br />cost. Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the Final Change Order
<br />and the Project Completion Affidavit were approved subject to the filing ofthe appropriate three-year
<br />Maintenance Bond.

<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manger has submitted the Final Change
<br />Order on behalf of Rieth Riley Construction, 25200 State Road 23, South Bend, Indiana, indicating
<br />that the contract amount be decreased by $5,746.07 for a new contract sum including this Change
<br />Order in the amount of $293,890.43. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost. Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried,
<br />the Final Change Order and the Project Completion Affidavit were approved subject to the filing of
<br />the appropriate three-year Maintenance Bond.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - MORRIS
<br />PERFORMING ARTS CENTER - BOX OFFICE REMODELING - PROJECT NO. 100-85
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final Change
<br />Order with no monetary change for a new contract sum including this Change Order in the amount
<br />of $25,000.00. Additionally submitted was the Project Completion Affidavit indicating this new final
<br />cost. Upon a motion made by Mr. Inks, seconded by Mr. Gilot and carried, the Final Change Order
<br />and the Project Completion Affidavit were approved.
