<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - 2001
<br />PUBLIC WORKS PARTNERSHIP PROGRAM - ZONE 2 - SELGE CONSTRUCTION
<br />COMPANY, INC- - PROJECT NO 101-015
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final Change
<br />Order, on behalf of Selge Construction Company, Inc., 2833 indicating that the contract amount be
<br />decreased by $58,160.21 for a new contract sum including this Change Order in the amount: of
<br />$174,914.79. Additionally submitted was the Proj ect Completion Affidavit indicating this new final
<br />cost. Upon a motion made by Mr. Littrell, seconded by Mr. Inks and carried, the Final Change Order
<br />and the Project Completion Affidavit were approved subject to the filing of the appropriate three-
<br />year Maintenance Bond.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - DUBAIL
<br />STREET AND DOUGLAS STREET IMPROVEMENTS - RIETH RILEY CONSTRUCTION
<br />COMPANY, INC. - PROJECT NO 101-026
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final Change
<br />Order on behalf of Rieth Riley Construction, Inc., Post Office Box 1775, South Bend, Indiana,
<br />indicating that the contract amount be decreased by $26,127.69 for a new contract sum including this
<br />Change Order in the amount of $127,497.46. Additionally submitted was the Project Completion
<br />Affidavit indicating this new final cost. Upon a motion made by Mr. Littrell, seconded by Mr. Gilot
<br />and carried, the Final Change Order and the Project Completion Affidavit were approved subject to
<br />the filing of the appropriate three-year Maintenance Bond.

<br />LINCOLNWAY WEST PHASE II - ELMER STREET TO LAFAYETTE STREET - RIETH RILEY
<br />CONSTRUCTION COMPANY INC. - PROJECT NO. 100-57
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final Change
<br />Order on behalf of Rieth Riley Construction Company, Inc., 25200 State Road 23, South Bend,
<br />Indiana, indicating that the contract amount be increased $84,181.45 for a new contract sum
<br />including this Change Order in the amount of $1,679,318.85. Additionally submitted was the Project
<br />final cost. Upon a motion made by Mr. Littrell, seconded
<br />Completion Affidavit indicating this new
<br />by Mr. Gilot and carried, the Final Change Order and the Project Completion Affidavit were
<br />approved subject to the fling of the appropriate three-year Maintenance Bond.

<br />CONSTRUCTION - PROJECT NO 101-015
<br />Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final Change
<br />Order on behalf of Kaser Spraker Construction, 2548 West State Road 2, South Bend, Indiana,
<br />indicating that the contract amount be decreased by $63,405.90 for a new contract sum including this
<br />Change Order in the amount of $219,469.10. Additionally submitted was the Project Completion
<br />Affidavit indicating this new final cost. Upon a motion made by Mr. Inks, seconded by Mr. Littrell
<br />and carried, the Final Change Order and the Project Completion Affidavit were approved subject to
<br />the filing of the appropriate three-year Maintenance Bond.
<br />APPROVE THE CERTIFICATION OF SUBSTANTIAL COMPLETION AND CERTIFICATION
<br />OF FINAL COMPLETION - LABORATORY CASEWORK PROJECT - WASTEWATER
<br />TREATMENT PLANT KASER SPRAKER CONSTRUCTION - PROJECT NO. 100-088
<br />Mr. Gilot advised that Mr. Jack Dillon, Environmental Services has submitted the Certification of
<br />Substantial Completion and Certification of Final Completion on behalf of Kaser Spraker
<br />Construction, Post Office Box 3605, South Bend, Indiana, indicating that the final contract amount
<br />is $112,973.00. Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the
<br />Substantial Completion and Certification of Final Completion were approved.
<br />APPROVAL OF PROPOSAL - ENGINEERING DESIGN SERVICES - REHABILITATION OF
<br />FOUR (4) BELT FILTER PRESS - WASTEWATER TREATMENT PLANT DLZ INDIANA
<br />Mr. Gilot stated that the Board is in receipt of a Proposal between the City of South Bend and DLZ,
<br />2211 East Jefferson, South Bend, Indiana, for the above referred to project. Mr. Gilot noted that the
<br />Proposal is in the amount of $3,650.00. Therefore, upon a motion made by Mr. Littrell, seconded
<br />by Mr. Gilot and carried, the Proposal was approved and executed.
