Post Office Box 145 Leo, Indiana 46765 Bid was signed by Ms. Kathy Jones Non -Collusion Affidavit was in order Non -Discrimination Commitment form was completed Five per cent (5%) Bid Bond was submitted NOVEMBER 26, 2001 BID: $112,082.70 Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the above bids were referred to the Division of Engineering for review and recommendation. APPROVE MINUTES OF PREVIOUS MEETING Upon a motion made by Mr. Inks, seconded by Mr. Gilot and carried, the minutes of the regular meeting of the Board held on November 13, 2001, were approved. APPROVE REQUEST TO ADVERTISE FOR RECEIPT OF BIDS - SALE OF ABANDONED VEHICLES Ms. Kathy Davis, Director, Department of Code Enforcement, requested permission to advertise for the sale of approximately eighteen (18) abandoned vehicles, which are being stored at Super Auto Salvage Corporation, 3300 South Main Street, South Bend, Indiana. It was noted that all vehicles have been stored more than fifteen (15) days, identification checks had been run for auto theft and the owners and lienholders notified. Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the above request was approved. APPROVE CHANGE ORDER - 2001 CONCRETE PAVEMENT REPAIRS - LAPORTE CONSTRUCTION COMPANY, INC. - PROJECT NO 101-053 Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order No. 2 on behalf of LaPorte Construction Company, Inc., Post Office Box 577, LaPorte, Indiana indicating that the Contract amount be increased $33,385.78 for a new Contract sum including this Change Order in the amount of $245,156.89. Upon a motion made by Mr. Littrell, seconded by Mr. Inks and carried, the Change Order was approved. APPROVE CHANGE ORDER - ROBERT BOSCH CORPORATION - FACILITY IMPROVEMENTS - PHASE I - KASER SPRAKER CONSTRUCTION - PROJECT NO. 101-046 Mr. Gilot advised that Mr. Toy Villa, Public Construction Manger, has submitted Change Order No. 3 on behalf of Kaser Spraker Construction, Post Office Box 3605, South Bend, Indiana indicating that the Contract amount be increased $1,699,00 for a new Contract sum including this Change Order in the amount of $1,909,056.00. Upon a motion made by Mr. Littrell, seconded by Mr. Inks and carried, the Change Order was approved.
