<br />APPROVAL `OF AMENDMENT TO AGREEMENT - PROFESSIONAL CONSULTING
<br />SERVICES - PHASE II ENVIRONMENTAL SITE ASSESSMENT OF AN AREA IN
<br />STUDEBAKER CORRIDOR - HULL & ASSOCIATES, INC.
<br />Mr. Gilot stated that the Board is in receipt of an Agreement between the City of South Bend and
<br />Hull & Associates, Inc., 6130 Wilcox Road, Dublin, Ohio, for additional Level IV analytical
<br />reporting, installation of additional monitoring wells, a transition form the original VRP to RISC
<br />guidelines and surveying costs to address inaccuracies in the maps that were provided to Hull. Mr.
<br />Gilot noted that the Agreement is in the amount of $23,000.00. Therefore, upon a motion made by
<br />Mr. Gilot, seconded by Mr. Inks and carried, the Agreement was approved and executed.
<br />APPROVAL OF LICENSE APPLICATION FOR MASSAGE ESTABLISHMENT - MICHAEL
<br />& COMPANY, INC.
<br />Mr. Gilot stated that the Board is in receipt of favorable recommendations from the Fire Department,
<br />Police Department, Building Department and the Legal Department for a License Application for
<br />a Massage Establishment, as submitted by Mr. Michael Murray, 521 East Jefferson South Bend,
<br />Indiana, for a renewal of a License at 521 East Jefferson, South Bend, Indiana. Upon a motion made
<br />by Mr. Littrell, seconded by Mr. Inks and carried, the License Application was approved.
<br />APPROVE LICENSE APPLICATION - PUBLIC PARKING FACILITY
<br />Upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried, the following license
<br />application was approved:
<br />Valley American Limited Partnership
<br />211 West Washington
<br />South Bend, Indiana 46601
<br />The above licence application was approved and referred to the Deputy Controller's Office for
<br />issuance of a license. It was noted that the required Proof of Insurance as well as the hours and rates
<br />to be charged was attached to the application. Favorable recommendations were received from the
<br />Fire Department, Code Enforcement and the Legal Department.
<br />APPROVAL OF RECOMMENDATION - TO CONDUCT SUNBURST MARATHON
<br />Mr. Gilot stated that favorable recommendations have been received by the Division of Traffic &
<br />Lighting, Police Department, Park Department and the City Attorney's office, in reference to a
<br />request as submitted by Ms. Liz Peralta, Memorial Health & Lifestyle, 111 West Jefferson, South
<br />Bend, Indiana to conduct the above referred to Sunburst Marathon on June 8, 2002, on the
<br />designated route as submitted. Upon a motion made by Mr. Littrell, seconded by Mr. Inks and
<br />carried, the recommendation was approved.
<br />APPROVAL OF RECOMMENDATION - TO CONDUCT MICHIANA AIDS WALK 2002
<br />Mr. Gilot stated that favorable recommendations have been received by the Division of Engineering,
<br />Park Department, Police Department and the City Attorney's office, in reference to a request as
<br />submitted by Ms. Holly McCune, AIDS Ministry, Post Office Box 11582, South Bend, Indiana to
<br />conduct the above referred to walk on September 22, 2002, on the designated route as submitted.
<br />Upon a motion made by Mr. Littrell, seconded by Mr. Inks and carried, the recommendation was
<br />approved.
<br />APPROVAL OF RECOMMENDATION - TO CONDUCT RIVERSIDE RUN
<br />Mr. Gilot stated that favorable recommendations have been received by the Division of Engineering,
<br />Park Department, Police Department and the City Attorney's office, in reference to a request as
<br />submitted by Ms. Susan O'Connor, Park Department to conduct the above referred to run on July
<br />6, 2002, on the designated route as submitted. Upon a motion made by Mr. Inks, seconded by Mr.
<br />Littrell and carried, the recommendation was approved.
<br />APPROVAL OF RECOMMENDATION - TO CONDUCT "HIGHER CALL" PRAYER VIGIL
<br />Mr. Gilot stated that favorable recommendations have been received by the Division of Engineering,
<br />Police Department and the City Attorney's office, in reference to a request as submitted by Mr. John
<br />Wilson, Higher Call, 3717 South Michigan, South Bend, Indiana, to conduct the above referred to
<br />prayer vigil on the steps and sidewalk in front of Lafayette Street by the old Court House, on May
<br />
