<br />:EA "A" DEMOLITION PHASE
<br />The Board, having discussed the bid extension of time for the above -referred to project at
<br />its Agenda Session on October 6, 2005, ratified the decision to invite all bidders to extend
<br />their bids for sixty (60) days. Upon a motion by Mr. Inks, seconded by Mr. Littrell and
<br />carried, the decision for a sixty (60) day bid extension was ratified.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT —
<br />2005 CURB AND SIDEWALK PROGRAM — ZONE 1 — L. L GEANS
<br />CONSTRUCTION — PROJECT NO. 105-002 (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />on behalf of L.L. Geans Construction, 1923 North Home Street, Mishawaka, Indiana,
<br />indicating that the Contract amount be decreased $43,297.85 for a new Contract sum
<br />including this Change Order in the amount of $216,502.15. Additionally submitted was
<br />the Project Completion Affidavit indicating this new final cost. Upon a motion made by
<br />Mr. Littrell, seconded by Mr. Inks, and carried, the Final Change Order and Project
<br />Completion Affidavit was approved.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT
<br />2005 CURB AND SIDEWALK PROGRAM — ZONE 2 - L. L GEANS
<br />CONSTRUCTION —PROJECT NO. 105-002 (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />on behalf of L.L. Geans Construction, 1923 North Home Street, Mishawaka, Indiana,
<br />indicating that the Contract amount be decreased $24,940.86 for a new Contract sum
<br />including this Change Order in the amount of $214,525.14. Additionally submitted was
<br />the Project Completion Affidavit indicating this new final cost. Upon a motion made Mr.
<br />Littrell, seconded by Mr. Inks, and carried, the Final Change Order and Project
<br />Completion Affidavit was approved.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT —
<br />PORTAGE AVENUE STREETSCAPE — KING STREET — RAM CONSTRUCTION —
<br />PROJECT NO. 105-024A (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />on behalf of Ram Construction, 1957 Denslow, South Bend, Indiana, indicating that the
<br />Contract amount be decreased $1,844.56 for a new Contract sum including this Change
<br />Order in the amount of $25,536.44. Additionally submitted was the Project Completion
<br />Affidavit indicating this new final cost. Upon a motion made by Mr. Inks, seconded by
<br />Mr. Littrell, and carried, the Final Change Order and Project Completion Affidavit was
<br />approved.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT
<br />PORTAGE AVENUE INTERSECTION IMPROVEMENTS — HUMBOLDT STREET —
<br />RAM CONSTRUCTION — PROJECT NO. 105-023A (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />on behalf of Ram Construction, 1957 Denslow, South Bend, Indiana, indicating that the
<br />Contract amount be decreased $4,285.96 for a new Contract sum including this Change
<br />Order in the amount of $28,285.71. Additionally submitted was the Project Completion
<br />Affidavit indicating this new final cost. Upon a motion made by Mr. Inks, seconded by
<br />Mr. Littrell, and carried, the Final Change Order and Project Completion Affidavit was
<br />approved.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT —
<br />WESTERN AVENUE IMPROVEMENTS — RIETH-RILEY CONSTRUCTION CO.
<br />INC. — PROJECT NO. 104-026A (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />on behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend,
<br />Indiana, indicating that the Contract amount be decreased $21,034.35 for a new Contract
<br />sum including this Change Order in the amount of $327,333.75. Additionally submitted
<br />was the Project Completion Affidavit indicating this new final cost. Upon a motion made
<br />by Mr. Inks, seconded by Mr. Littrell, and carried, the Final Change Order and Project
<br />Completion Affidavit was approved.
