REGULAR MEETING JULY 25, 2005 " <br />APPROVE CHANGE ORDER NO. 2 (FINAL) AND CONSENT OF SURETY TO <br />FINAL PAYMENT — EDISON WATER TREATMENT PLANT RENOVATION — <br />PHASE I — KASER SPRAKER CONSTRUCTION — PROJECT NO. 103-050 (BOND) <br />Mr. Gilot advised that Mr. John Wiltrout, Water Works, has submitted Change Order No. <br />1 (Final) indicating that the contract amount be decreased by $3,587.00 for a new <br />Contract sum including this Change Order in the amount of $1,155,112.00. Additionally <br />submitted was the Project Completion Affidavit indicating this new final cost. Upon a <br />motion made by Mr. Littrell, seconded by Mr. Gilot, and carried, Change Order No. 1 <br />(Final) and the Project Completion Affidavit were approved subject to the filing of the <br />appropriate three-year Maintenance Bond. <br />APPROVE PROJECT COMPLETION AFFIDAVIT — 12TH AND 13TH FLOOR <br />CARPETING — COUNTY CITY BUILDING — MIDWEST TIRE & INTERIORS — <br />PROJECT NO. 105-048C (GENERAL FUND) <br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion <br />Affidavit on behalf of Midwest Tire & Interiors, 1805 North Cedar Street, Mishawaka, <br />Indiana, for the above referred to project, indicating a final cost of $34,012.00.
