<br />AWARD QUOTATION — WESTERN AVENUE MARKETING BROCHURE
<br />(COIT/WESTERN AVENUE FUNDS)
<br />Mr. David Fecteau, Community & Economic Development, advised the board that on
<br />June 27, 2005, quotations were received and opened for the above referred to project.
<br />After reviewing those quotations, Mr. Fecteau recommends that the Board award the
<br />contract to the lowest, responsive and responsible bidder, Van Paris & Associates, 55380
<br />Fresno Street, South Bend, Indiana, in the amount of $4,725.00. Therefore, Mr. Littrell
<br />made a motion that the recommendation be accepted and the quotation be awarded as
<br />outlined above. Mr. Gilot seconded the motion, which carried.
<br />APPROVE CHANGE ORDER — NEW CENTRAL FIRE STATION — CASTEEL
<br />CONSTRUCTION — PROJECT NO. 103-020 (BOND)
<br />Mr. Gilot advised that Mr. Carl Littrell, Engineering, has submitted Change Order No. 1
<br />on behalf of Casteel Construction, 23186 West Ireland Road, South Bend, Indiana,
<br />indicating that the Contract amount be increased $144,309.52 for a new Contract sum
<br />including this Change Order in the amount of $16,532,809.52. Upon a motion made by
<br />Mr. Littrell, seconded by Mr. Gilot, and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER — NEW POLICE DEPARTMENT HEADQUARTERS —
<br />CASTEEL CONSTRUCTION — PROJECT NO. 103-020 (BOND)
<br />Mr. Gilot advised that Mr. Carl Littrell, Engineering, has submitted Change Order No. 2
<br />on behalf of Casteel Construction, 23186 West Ireland Road, South Bend, Indiana,
<br />indicating that the Contract amount be increased $406,665.00 for a new Contract sum
<br />including this Change Order in the amount of $16,939,474.52. Upon a motion made by
<br />Mr. Gilot, seconded by Mr. Littrell, and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDERS — SOUTH BEND FIRE DEPARTMENT STORAGE
<br />BUILDING — H.G. CHRISTMAN CONSTRUCTION — PROJECT NO. 105-036
<br />(EMS/COIT)
<br />Mr. Gilot advised that Mr. Jim Freeman, Engineering, has submitted Change Order No. 1
<br />and No. 3 on behalf of H.G. Christman Construction, 850 South Fellows, South Bend,
<br />Indiana. Change Order No. 1 indicates that the Contract amount be increased $3,913.00
<br />for a new Contract sum including this Change Order in the amount of $325,913.00.
<br />Change Order No. 3 indicates no increase in cost. Upon a motion made by Mr. Gilot,
<br />seconded by Mr. Littrell, and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION
<br />AFFIDAVIT — SHERMAN AVENUE IMPROVEMENTS — ROSE STREET TO
<br />BEALE STREET — PROJECT NO. 105-029 (GENERAL FUND)
<br />Mr. Gilot advised that Mr. Carl Littrell, Engineering, has submitted Change Order No. 1
<br />(Final) indicating that the contract amount be decreased by $1,827.22 for a new contract
<br />sum including this Change Order in the amount of $73,751.53. Additionally submitted
<br />was the Project Completion Affidavit indicating this new final cost. Upon a motion made
<br />by Mr. Littrell, seconded by Mr. Gilot, and carried, Change Order No. 1 (Final) and the
<br />Project Completion Affidavit were approved subject to the filing of the appropriate three-
<br />year Maintenance Bond.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION
<br />AFFIDAVIT — MISHAWAKA AVENUE STREETSCAPES — IRONWOOD TO 25TH
<br />STREET — TRANS TECH ELECTRIC — PROJECT NO. 104-014A (GENERAL FUND)
<br />Mr. Gilot advised that Mr. Littrell, Engineering, has submitted Change Order No. 1
<br />(Final) indicating that the contract amount be decreased by $19,936.40 for a new contract
<br />sum including this Change Order in the amount of $88,066.60. Additionally submitted
<br />was the Project Completion Affidavit indicating this new final cost. Upon a motion made
<br />by Mr. Littrell, seconded by Mr. Gilot, and carried, Change Order No. 1 (Final) and the
<br />Project Completion Affidavit were approved subject to the filing of the appropriate three-
<br />year Maintenance Bond.
