<br />Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, all bids were
<br />rejected for this project. It was noted that this project will not be constructed at this time.
<br />APPROVE CHANGE ORDER — 12TH AND 13TH FLOOR CARPETING — COUNTY

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on
<br />behalf of Midwest Tile & Interiors, 1805 North Cedar, Mishawaka, Indiana, indicating
<br />that the Contract amount be increased $600.00 for a new Contract sum including this
<br />Change Order in the amount of $34,012.00. Upon a motion made by Mr. Gilot, seconded
<br />by Mr. Littrell, and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER — BENDIX RELOCATION — BROOKS
<br />CONSTRUCTION COMPANY — PROJECT NO. 102-037 (AIRPORT TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 5 on
<br />behalf of Brooks Construction, 625 Beiger, Mishawaka, Indiana, indicating that the
<br />Contract amount be increased $72,828.55 for a new Contract sum including this Change
<br />Order in the amount of $1,395,486.76. Upon a motion made by Mr. Inks, seconded by
<br />Mr. Littrell, and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER — 2004/2005 SEWER LINING PROGRAM
<br />INSITUFORM TECHNOLOGIES, INC. — PROJECT NO 104-062 (SEWER WORKS)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on
<br />behalf of Insituform Technologies, Inc. 12897 Main Street, Lemont, Illinois, indicating
<br />that the Contract amount be increased $73,370.00 for a new Contract sum including this
<br />Change Order in the amount of $1,048,280.00. Upon a motion made by Mr. Inks,
<br />seconded by Mr. Littrell, and carried, the Change Order was approved.
<br />APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT —
<br />PORTAGE AVENUE GATEWAY STREETSCAPES — SELGE CONSTRUCTION —
<br />PROJECT NO. 104-013 (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />indicating that the contract amount be increase $224.30 for a new contract sum including
<br />this Change Order in the amount of $177,617.30. Additionally submitted was the Project
<br />Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Gilot,
<br />seconded by Mr. Littrell, and carried, the Final Change Order and the Project Completion
<br />Affidavit were approved subject to the filing of the appropriate three-year Maintenance
<br />Bond.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Final Change Order
<br />on behalf of Northern Construction Services Corporation, Post Office Box 1299, Niles,
<br />Michigan, indicating that the contract amount be decreased by $3,779.90 for a new
<br />contract sum including this Change Order in the amount of $75,138.35. Additionally
<br />submitted was the Project Completion Affidavit indicating this new final cost. Upon a
<br />motion made by Mr. Inks, seconded by Mr. Littrell, and carried, the Final Change Order
<br />and the Project Completion Affidavit were approved subject to the filing of the
<br />appropriate three-year Maintenance Bond.

<br />JEFFERSON STREET ENTRANCE APPROACH — RIETH RILEY CONSTRUCTION
<br />— PROJECT NO. 105-057 (GENERAL FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Rieth Riley Construction, 25200 State Road 23, South Bend,
<br />Indiana, for the above referred to project, indicating a final cost of $6,750.00. Upon a
<br />motion made by Mr. Gilot, seconded by Mr. Inks, and carried, the Project Completion
<br />Affidavit was approved.
