Background Data: <br /> <br />a. <br /> <br />Effective Date of Owner-Engineer Agreement: <br /> <br />June 14, 2022 <br /> <br />b. <br /> <br />Owner: <br /> <br />Board of Public Works, City of South Bend, IN <br /> <br />c. <br /> <br />Engineer: <br /> <br />A&Z Engineering, LLC <br /> <br />d. <br /> <br />Project: <br /> <br />Grand Trunk Watermain Replacements at Greenlawn Avenue and S. 30th <br />Street Crossings/ SB Project # 122-041 <br /> <br />2. Description of Modifications: Modifications reflect revisions to previously reviewed plans <br />by G&T Western Railroad (GRR) for Watermain Replacements at Greenlawn Avenue. <br />Modifications include changing the size of the casing and carrier pipes, incorporating <br />ChoiceLight utility conduit in the plans, resubmitting permit applications, coordination <br />between Owner, Contractor, ChoiceLight and other utilities and GRR, undertake GRR <br />review compliance and construction support and coordination according to Canadian <br />National Utility Crossing/Encroachment Application Packet for Greenlawn Avenue. <br /> <br />EXHIBIT A <br /> <br />The Scope of Services authorized to be performed by Engineer in accordance with the <br />Agreement, previous amendment number 1, is modified as follows: <br /> <br /> <br />PART 1 – BASIC SERVICES <br /> <br /> A1.03a Post-Final Design Phase (100% Plans without Bidding Documents) <br /> <br />A. Engineer shall, <br /> <br />1. Revise the 100% plans to include, <br />a. Casing and carrier pipe size revisions and details <br />2. Coordinate with Owner, Utilities, Contractor and Field Monitoring <br />Team <br />3. Submit revised plans to Owner for review comments <br />4. Discuss review comments and compliance. <br />5. Provide construction support and coordination <br />
