T. Lee Engineering, Inc.

Post Office Box 1260
Greenwood, Indiana 46142-1260
Phone: 317-502-9634
April 22, 2025

Ms. Christina Bracken
Erie Insurance Group
7609 West Jefferson Street, Suite 200
Fort Wayne, Indiana 46804

Re: Residential Property Structural Evaluation
Claim No: A00006677171
Insured: Mr. Alexander Horvath
Subject: Report of Findings
File No: L03205

Dear Ms. Bracken:
Mr. Alexander Horvath reported that his property was damaged by a tree and tree
limbs impacts on March 30, 2025. The residential property was located at 13546
Brick Road in Granger, Indiana.

T. Lee Engineering, Inc. was retained by Erie Insurance Group to evaluate the
reported damage to the residential structure. We were specifically asked to
determine the extent of damage to the building caused by tree and tree limbs
impacts. Mr. Timothy A. Lee, P.E. performed our study of the property on April 5,
2025. Mr. and Mrs. Horvath were present during the site study to provide first-
hand information.

CONCLUSIONS
1. The physical evidence observed at the property indicated the tree and tree
limbs impacts resulted in localized damage to the south
and west-facing roof slopes, the west-facing roof slope of the first story,
the asphalt composition shingles, and the gypsum wallboard ceiling of the
master bedroom and kitchen.
2. The physical evidence observed at the attached garage indicated the tree
and tree limbs impacts resulted in widespread damage to the east and
west-facing roof slopes, the wood framing of the roof structure, asphalt
composition shingles, the entry door and overhead garage door, and the
south and east concrete masonry unit (CMU) exterior walls, and the upper
half of the west CMU exterior wall.
