All rights reserved. <br /> <br />This is EXHIBIT A, consisting of 14 pages, referred to in <br />and part of the Agreement between Owner and Engineer <br />for Professional Services dated , 2025. <br />Engineer’s Services <br /> <br />Article 1 of the Agreement is supplemented to include the following agreement of the parties. <br /> <br />Engineer shall provide Basic and Additional Services as set forth below. <br /> <br />PART 1 – BASIC SERVICES <br />Description of Projects <br />Michigan Street (Old US 31) HMA Overlay will occur along 3,400 feet of Michigan Street from the City’s <br />southern limit to the south side of Kern Road. Improvements will include the milling of existing asphalt <br />pavement and overlaying with new asphalt pavement. New pavement markings reducing the roadway <br />section for 4-lanes to 3-lanes to match the section immediately south of the City limits. No Survey or Utility <br />Coordination services will be conducted. Project will utilize INDOT Community Crossing Matching Grant <br />(CCMG) funds. <br />Riverside Drive HMA Pavement Reconstruction will occur along 2,350 feet of Riverside Drive from <br />south side of Boland Drive to south side of Cleveland Road. Improvement will include the removal of the <br />existing asphalt pavement and subgrade and replacing with new subgrade and asphalt pavement. Limited <br />Survey services will be conducted. Utility coordination with no relocations anticipated. Project will utilize <br />INDOT CCMG funds. <br />Hillside Avenue HMA Pavement will occur along 250 feet of currently unpaved Hillside Avenue from <br />Olive Street approach to Phillipa Street. Improvements will include construction of a new asphalt roadway. <br />Full survey and Utility Coordination. Project will utilize City funds. <br />Highland Avenue HMA Pavement will occur along 250 feet of currently unpaved Highland Avenue from <br />Olive Street approach to Phillipa Street. Improvements will include construction of a new asphalt roadway. <br />Full survey and Utility Coordination. Project will utilize City funds. <br />Assumptions: <br />1. All project related construction activities will be completed within existing right-of-way. <br />2. The only permit required for the project will be the IDEM Construction Stormwater General Permit. <br />3. Existing drainage infrastructure has adequate capacity to accommodate all project related drainage. <br /> <br />A1.01 Study and Report Phase <br />A. Engineer shall: <br />1. Consult with Owner to define and clarify Owner’s requirements for the Project and available <br />data.
