SINCLLAKEWOOAIR 54 <br />176 E.HoRECREATI09N <br />5. The Supervisor shall keep on the project site a first -aid kit supplied <br />according to current regulations. <br />6. The use of illegal drugs or alcohol while performing work on the project is <br />strictly prohibited. All employees will be drug tested based on requirements <br />set form in IC 4-13-18-5 or IC 4-13-18-6. <br />7. All employees, visitors and management shall wear an approved hard hat <br />while on site if the site requires the use of hard hats. <br />8. All employees shall wear durable work boots — no tennis shoes are allowed. <br />9. Sleeved shirts, minimum of four inches, and long pants are required <br />minimum clothing. <br />IO.Earphones are not allowed <br />11.Loud music is not allowed <br />12.All scaffolding must be checked daily and before each use for safety <br />compliance. <br />13.All tarps and blankets shall be made of fire retardant materials. <br />14.No smoking at any time on any job for any reason <br />15.All dozers, loaders, tractors and end loader backhoes must have functional <br />backup alarms at all times. <br />16.Cell phones: <br />a. Personal cell phones are not to be used on work sites except: <br />i. To report an emergency <br />ii. On approved break time <br />b. Use of business cell phones must not interfere with jobsite safety. <br />c.
