1. Designated Collection Sites <br />The company will utilize an approved collection site which will have the <br />personnel, materials, equipment, facilities and supervision necessary to provide <br />for the collection, security, temporary storage and shipping of urine specimens to <br />a DHHS-certified laboratory for testing. <br />D s�itlgpated Collcctioii Sites for this 1)larr are: <br />SEE APPENDIX C <br />2. SuLyisory 'ollectjorrs <br />Unless it is impractical for any other individual to perform this function, a direct <br />supervisor of an employee shall not serve as the collection site person for a test of <br />the employee. <br />3. Individual ['V ya Y <br />Collection procedures allow urine specimens to be provided by the individual in <br />private, unless there is reason to believe that the individual may alter or substitute <br />the specimen, as set forth below. <br />a. C irwCunnsttlpces REQUIRING Direct Observation <br />A second specimen of urine will be obtained as soon as possible under the <br />direct observation of a same gender collection site person whenever there <br />is reason to believe that a particular donor has altered or substituted the <br />specimen under the following circumstances. <br />(1) The donor has presented a specimen which falls outside the <br />allowable temperature ranges (32 degrees - 38 degrees C/90 <br />degrees - 100 degrees F.), and <br />(a) The donor declines to provide a measurement of oral body <br />temperature, or <br />(b) The donor's oral body temperature varies by more than 1 <br />degree C/1.8 degrees F from the temperature of the <br />specimen. <br />(2) The collection site person observes donor conduct clearly and <br />unequivocally indicating an attempt to substitute or adulterate the <br />specimen. <br />Aegion - Substance Abuse Policy & Procedures 15 <br />
