APPENDIX A <br />SCOPE OF ADDITIONAL SERVICES <br />South Main Street to Lafayette Connector <br />Our Design Agreement was made on the basis of our proposal dated August 9, 2010 for the <br />First Project Stage. That Agreement was executed on August 23, 2010. The First and following <br />Second and Third Stages were illustrated on a map included as Attachment E of the <br />Agreement. Attachment E is included for reference and information. <br />The lengthy preliminary design process led to the project now under development as a Single <br />Project, which extends from Ireland Road to Chippewa Avenue and from Lafayette Boulevard to <br />Addison Street. Based upon the preliminary design, a Single Project can be constructed at less <br />cost and time than the original concept of a 3 -stage project completion. Economy of the scale <br />and scope of the project allows the City to utilize available funds for both Right -of -Way <br />acquisition and construction. Additional design effort and time is required to complete the <br />improvements as a single project. For comparison, a map of the current, Single Project is also <br />included (Attachment F). The following items are requested as effort not envisioned in our <br />August 2010 Proposal: <br />• Survey — <br />• Preliminary Design — <br />• Detail Design — <br />• Parcel Plats and Legal Descriptions — <br />Survey <br />Additional topographic survey was needed to include the roadways for the second and third <br />original phases. <br />1,200 additional lineal feet of roadway survey was needed. <br />Approximately 1 acre of land area was surveyed to account for grading plans needed <br />on the DeBuysser and City Park land for grading plans and the Barbie Street <br />Retention Basin. <br />Survey of the existing Storm Sewer on Chippewa from Main Street intersection west <br />to the old AM General Plant entrance. <br />Also, we have included a small contingency for more survey needs resulting from <br />preliminary design alternatives which are likely to be encountered. <br />Preliminary Design <br />More time and effort was needed in preliminary design <br />proper location of the intersection of Main Street and th e <br />roundabout was shifted once and then again to the <br />intersection.) <br />to account for public sentiment and a <br />East -West Collector Roadway.
