<br />Therefore, Ms. Maradik made a motion that the recommendation be accepted and the quotation
<br />be awarded as outlined above. Mr. Mueller seconded the motion, which carried.
<br />AWARD QUOTATION — 2018 PREVENTATIVE MAINTENANCE AND REPAIRS OF
<br />HVAC EQUIPMENT AND CONTROLS (UTILITES OTHER CONTRACTURAL
<br />SERVICES, AND BUILDING MAINTENANCE)
<br />Mr. Jacob Klosinski, Wastewater, advised the Board that on June 12, 2018, quotations were
<br />received and opened for the above referenced service. After reviewing those quotations, Mr.
<br />Klosinski recommended that the Board award the contract to the lowest responsive and
<br />responsible bidder, Ideal Consolidated, Inc., 806 West Sample Street, South Bend, IN 46601, in
<br />the amount of $65,821 for three (3) years, the Alternate Bid. Therefore, Ms. Maradik made a
<br />motion that the recommendation be accepted and the quotation be awarded as outlined above.
<br />Mr. Mueller seconded the motion, which carried.
<br />APPROVE CHANGE ORDER NO. 1 - IRONWOOD DRIVE PAVEMENT
<br />REHABILITATION AND INTERSECTION — PROJECT NO 117-071A (LOCAL ROAD
<br />AND BRIDGES GRANT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of C&E Excavating, Inc., 53767 County Road 9, Elkhart, IN 46514, indicating the contract
<br />amount be increased by $4,615.80 for a new contract sum, including this Change Order, in the
<br />amount of $1,352,208.30. Upon a motion made by Ms. Maradik, seconded by Mr. Mueller and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — 100 WAYNE STREET PLAZA AND STREETSCAPE
<br />- PROJECT NO. 116-047R (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Premium Concrete Services, Inc., 712 Richmond Street, Elkhart, IN 46516, indicating the
<br />contract amount be increased by $32,850 and forty-three (43) days, for a new contract sum,
<br />including this Change Order, in the amount of $256,960 and a new completion date of June 16,
<br />2018. Upon a motion made by Ms. Maradik, seconded by Mr. Mueller and carried, the Change
<br />Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — WEST RACE EMERGENCY REPAIR — PROJECT
<br />NO. 115-115 (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Mr.. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of HRP Construction Co., 5777 Cleveland Rd., South Bend, IN 46628, indicating the contract
<br />amount be increased by $9,150 and seven (7) days, for a new contract sum, including this
<br />Change Order, in the amount of $93,250 and a new completion date of June 1, 2018. Upon a
<br />motion made by Ms. Maradik, seconded by Mr. Mueller and carried, the Change Order was
<br />approved.
<br />APPROVE CHANGE ORDER NO. 1 — DOWNTOWN CROSS STREET IMPROVEMENTS —
<br />PROJECT NO. 117-041A (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Walsh & Kelly, 24358 State Road 23, South Bend, IN 46614, indicating the contract amount
<br />be increased by $10,799.20 for a new contract sum, including this Change Order, in the amount
<br />of $1,669,859.80. Upon a motion made by Ms. Maradik, seconded by Mr. Mueller and carried,
<br />the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— ALLEY DRYWELL INSTALL (NEAR ST VINCENT & NOTRE DAME AVE
<br />PROJECT NO. 117-101 (LRSA)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of John Boettcher Sewer & Excavating, 3505 N. Home St., Mishawaka, IN 46545,
<br />indicating the contract amount be increased by $500 for a new contract sum, including this
<br />Change Order, of $10,421. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $10,421. Upon a motion made by Ms. Maradik, seconded by Mr.
<br />Mueller and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved.
