<br />Topographic Survey and Right of Way Analysis
<br />Dear Mr. Zielinski:
<br />Lawson -Fisher Associates P.C, (LFA) is presenting a sub -contractor scope of work,
<br />manhour justification and scope limit exhibit based upon your request for proposal dated May 24,
<br />2017 and subsequent discussions regarding survey scope. These survey services will support
<br />American Structurepoint, Inc. (Structurepoint) as they respond to the City of South Bend, Indiana
<br />(CITY) request for professional design services pertaining to a proposed left turn lane at Sample
<br />Street and Main Street for eastbound traffic on Sample Street.
<br />As requested and attached to this letter, you will find a Scope of Services, a list of Owner
<br />Provided Data, a Proposed Schedule, Proposed Fee, Manhour Estimates (Attachments Al and
<br />A2), and scope limit exhibit (Attachment B), for submittal to the CITY for consideration.
<br />Scope of Services
<br />A. LFA shall provide:
<br />1. One time necessary topographic survey and utility representation, for
<br />roadway design purposes, within the areas indicated on the attached
<br />survey scope map.
<br />2. A recorded Location Control Route Surrey along Sample Street from
<br />Lafayette Boulevard to Main Street described below:
<br />All of the work above shall be in accordance with Indiana Code (I,C. 25-
<br />21.5); Indiana Administrative Code (865 I.A.C. 1-12), and the Design
<br />Manual, Indiana Department of Transportation, Part III, Location Surveys
<br />(Survey Manual). If there is any conflict between these guidance
<br />documents, the order of precedence shall be:
<br />a) I.C.25-21.5
<br />b) 865 I.A.C. 1-12, and
<br />c) Survey Manual
