Wastewater Treatment Plant Belt Filter Press #4 Rehabilitation <br />Project No. 117-025 <br />SECTION II: PLAN AND EQUIPMENT QUESTIONNAIRE <br />(1) Attach an explanation of your plan or layout for performing proposed work. <br />Background <br />Our plan to accomplish this work is based on our experience completing major projects and <br />critical shutdowns. Industrial Maintenance and Contract Services, L.P. {IMCLP} has been the <br />lead contractor for a major industrial plant in the South Bend area for over 25 years. During that <br />time, IMCLP has completed more than 25,000 tasks and successfully completed every task <br />assigned by the client. Also, during that time, IMCLP has completed 225 shutdowns of one - <br />week duration and approximately 500 shutdowns of one- to two-day duration. Without <br />exception, all of these shutdowns were completed on schedule and within budget. <br />Plan <br />Based on the scope of work, this project can be completed within two weeks. The following <br />plan includes the major steps to perform the work upon Notice to Proceed: <br />Pre -Construction Conference <br />IMCLP will participate in a pre -construction conference and provide the following documents in <br />advance of the meeting: progress schedule, schedule of submittals, and schedule of values. <br />Mobilization <br />Upon Notice to Proceed, IMCLP will provide one 40-foot storage trailer and office with electrical <br />power and heat for working personnel and material storage. A protected area by the trailers will <br />also be erected to store materials out of the weather. We will provide one service truck and one <br />crane truck to move the equipment to and from the job site. The office trailer has a restroom <br />and wash up facilities. <br />Project Staffing <br />IMCLP will provide a project manager, general foreman, safety and quality control manager, <br />accounting manager, and billing personnel. <br />Scheduling <br />A replacement schedule for worn parts in the belt filter press will be developed for the shutdown <br />of Belt Filter Press #4. <br />Progress Meetings <br />Daily progress meetings will be held during the shutdown. <br />Safety <br />Any new employee on this project will undergo a safety orientation before starting work. This <br />orientation includes all known hazards and problems involved in the completion of the work. <br />This orientation also includes safe procedures to be followed in completing the work. <br />IMCLP issues an Employee Handbook & Safety Requirements Manual. Also, IMCLP issues a <br />comprehensive IMCLP Safety Manual inclusive of OSHA rules. <br />
