This is EXHIBIT A, consisting of 13 pages, referred to in <br />and part of the Agreement between Owner and Engineer <br />for Professional Services dated December 22, 2016. <br />Services <br />Article 1 of the Agreement is supplemented to include the following agreement of the parties. <br />Engineer shall provide Basic and Additional Services as set forth below. <br />PART 1— BASIC SERVICES <br />A1.01 Project Description <br />A. The proposed project involves the rehabilitation of Edison Road between Duey Avenue and <br />Hickory Road and the rehabilitation of Ironwood Road between Mishawaka Avenue and McKinley <br />Avenue as identified in the City of South Bend's Community Crossings Application. The project <br />has a length of approximately 1.72 miles on Edison Road and 1.03 miles on Ironwood Road. The <br />project will include reconstruction of curb ramps which do not meet current ADA requirements, <br />asphalt pavement, pavement markings, and improvements at signalized intersections to include <br />FUR camera installation, controller & cabinet replacement, signal pole replacement, conduit <br />installation, and pedestrian signal improvements. It is anticipated the work will be completed within <br />existing right-of-way limits <br />AL02 Preliminary Design Phase <br />A. Upon written authorization from Owner, Engineer shall: <br />1. Prepare Preliminary Design Phase documents consisting of final design criteria, preliminary <br />drawings, outline specifications, and written descriptions of the Project. <br />2. Provide necessary field surveys and topographic and utility mapping for design purposes. <br />Survey to be completed within 45 calendar days after receipt of Notice to Proceed. Full <br />topographic survey of all quadrants of intersections listed below to collect data for <br />reconstruction of existing curb ramps. Survey shall include sidewalk area at each intersection <br />quadrant along Edison Rd. and Ironwood Rd. corridors noted above, extending in length <br />approximately 50 feet from the existing curb ramps. Survey will also locate the field marked <br />utilities and details of storm sewer structures within said intersection: <br />a. Ironwood Road Corridor at: <br />1) <br />Mishawaka Avenue <br />2) <br />Wall Street <br />3) <br />Hastings Street <br />4) <br />Jefferson Blvd <br />5) <br />Wilson Avenue <br />6) <br />Washington Street <br />7) <br />Colfax Avenue <br />b.
