APPENDIX 2 <br />SCOPE OF WORK <br />Proiect Understandin <br />The rain event that occurred overnight on Monday, August 15, 2016 resulted in unprecedented <br />storm runoff through the metal pipe arch for the Chippewa Avenue at Bowman Creek crossing in <br />South Bend, Indiana and additional runoff from the roadway, around the headwall and into the <br />creek. This storm runoff caused significant scour at and around the south headwall and between the <br />headwall and the south edge of the Chippewa Avenue pavement, exposing underground utilities, <br />foundations of overhead electric poles, and undercutting the pavement by approximately four (4) <br />feet for approximately thirty (30) feet. The City has placed traffic restrictions along the Chippewa <br />Avenue and has requested that plans be developed to address the emergency restoration for this <br />area. <br />Professional services provided will be two -fold. The first contract documents for securing quotes <br />for the pavement and roadside restoration. The second phase will be an investigation phase to <br />assess storm runoff, structure size(s) and potential long term remedial options. <br />PARTI.• SCOPE OFSERVICES. <br />A. Construction Documents <br />I . Identify required repairs to pavement section and eroded area between the pavement <br />and the south headwall. <br />2. Coordinate with utilities regarding location and scope of repairs near existing <br />facilities. <br />3. Coordinate the extent of debris removal from the Chippewa Avenue structure and the <br />smaller contributing structure from the east and include in the construction <br />documents. <br />4. Develop quote package with repairs and limits indicated on an aerial drawing and in <br />supplemental figures as required. <br />5. Coordinate with Indiana Department of Natural Resources (IDNR) regarding the <br />emergency nature of the repairs and the location within the Bowman Creek <br />floodway. <br />6. Deliver 10 copies of the complete quote package to the City for solicitation of <br />quotes. <br />B. Construction Administration <br />l . Coordinate kickoff meeting <br />2. Perform utility coordination through construction <br />3. Daily inspection as required (assumed 2 hrs/day for 2 weeks) <br />4. Review and process pay estimates and change orders <br />S. Coordinate final walk through <br />6. Develop punch list <br />7.
