REGULAR MEETING JUNE 6, 1994 <br />APPROVE CHANGE ORDER #p4 - CONTROL INFORMATION AND COMMUNICATION <br />SYSTEM - WASTEWATER TREATMENT PLANT <br />Mr. Leszczynski advised that Koontz -Wagner Electric Company, Inc., <br />3801 Voorde Drive, South Bend, Indiana, has submitted Change Order <br />No. 4 ',indicating that the Contract amount be increased by <br />$1,60Q.00 for a new Contract sum, including this Change Order, in <br />the amount of $1,138,953.32. Upon a motion made by Mrs. Manier, <br />seconded by Mr. Leszczynski and carried, the Change order was <br />approved. <br />- PUMP NO. 3 DRIVE REPLACEMENT - <br />Shaum Electric Company, Inc., 1125 Nappanee Street, Elkhart, <br />Indiana, has submitted Change Order No. 3 for the above referred <br />to project indicating a decrease of $2,582.00 for spare parts that <br />were returned as a result of changing from a chain to a belt drive <br />mechanism. The total amount of this Contract, including this <br />ChangelOrder is now $270,804.00. Upon a motion made by Mrs. <br />Manier!, seconded by Mr. Leszczynski and carried, the Change Order <br />was approved. <br />APPROVE CHANGE ORDERS - TWYCKENHAM DRIVE PROJECT <br />Mr. Jo Bellina, Division of Engineering, submitted to the Board <br />two (2 Change Orders for the above referred to project as <br />follow : <br />crease: $46,266.04 <br />scription: Pavement underdrains are called out in the <br />plans. Existing soil conditions from Rockne <br />Drive to South Bend Avenue do not require <br />subdrains. <br />Decrease: $32,279.10 <br />D scription: The traffic signal at Twyckenham and South Bend <br />Avenue was deleted by the LaPorte District <br />Traffic Department. It was decided that the <br />intersection did not meet warrants. The <br />handholes and conduit will be installed per <br />plans so when warrants are met the intersection <br />will not need to be reworked entirely. <br />Mrs. M nier made a motion that these Change Orders be approved. <br />Mr. Le;zczynski seconded the motion which carried. <br />AWARD BIDS - ONE LOADER AND ONE VAN <br />In a letter to the Board, Mr. Matthew L. Chlebowski, Director, <br />Division of Equipment Services, noted that he has reviewed the <br />bids received on May 23, 1994, and at this time recommends that <br />the Board award the low bid of MacDonald Machinery Company, Inc., <br />3911 Limestone Drive, Fort Wayne, Indiana, in the amount of <br />$135,157.00 for one Case Model 921 loader, with A/C, which <br />includes the trade-in of one (1) 1986 J.D. 644. <br />Additionally, Mr. Chlebowski recommended that the Board award the <br />low bid of Jordan Ford, 609 East Jefferson, Mishawaka, Indiana, in <br />the amount of $13,760.00 for one (1) 1994 Ford E150 Van. <br />Mrs. Ma ier made a motion that these recommendations be accepted <br />and the bids be awarded.
