<br />APPROVE CONSTRUCTION CONTRACT - EAST WAYNE STREET HISTORIC STREET
<br />LIGHTING UPGRADE - PROJECT NO. 98-56
<br />Mr. Caldwell advised that in accordance with the bid awarded on February 22, 1999, to Trans Tech
<br />Electric, Inc., 4601 Cleveland Road, South Bend, Indiana, in the amount of $155,000.00 for the
<br />above referred to project, a Contract in said amount was being submitted for Board approval. Upon
<br />a motion made by Ms. Roemer, seconded by Mr. Powell and carried, the Contract was approved and
<br />the appropriate Certificate of Insurance, Performance Bond and Labor and Materials Payment Bond
<br />as submitted were filed.
<br />APPROVAL OF PURCHASE AGREEMENT - 611 KALORAMA
<br />Mr. Caldwell stated that the Board is in receipt of an Agreement between the City of South Bend and
<br />Mr. Richard D. Fox, 821 South St. Louis Boulevard, South Bend, Indiana, for purchase of the City
<br />owned property at 611 Kalorama. Mr. Caldwell noted that the Agreement is in the amount of
<br />$427.00. Therefore, upon a motion made by Ms. Roemer, seconded by Mr. Powell and carried, the
<br />Agreement was approved and executed.
<br />APPROVAL OF AGREEMENT - MEDICAL SERVICES AGREEMENT - OCCUPATIONAL
<br />HEALTH NETWORK, LLC
<br />Mr. Caldwell stated that the Board is in receipt of an Agreement between the City of South Bend and
<br />Occupational Health Network, LLC, 801 East LaSalle Avenue, South Bend, Indiana, for City of
<br />South Bend Workers' Compensation. Mr. Caldwell noted that the Agreement states that the fees will
<br />not increase more than 5% in second and third years of the Agreement. Therefore, upon a motion
<br />made by Ms. Roemer, seconded by Mr. Powell and carried, the Agreement was approved and
<br />executed.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS - ONE (1 MORE
<br />OR LESS, FRONT END RUBBER TIRE LOADER
<br />In a memorandum to the Board, Mr. Matthew Chlebowski, Director, Central Services Division,
<br />requested permission to advertise for the receipt of bids for the above referred to equipment. Funding
<br />is provided by Organic Resources Capital Budget Fund. Therefore, upon a motion made by Ms.
<br />Roemer, seconded by Mr. Powell the request was a approved.
<br />FAVORABLE RECOMMENDATION - PETITION TO VACATE THE FIRST NORTH SOUTH
<br />ALLEY EAST OF SOUTH CHAPIN STREET
<br />Mr. Caldwell indicated that the Common Council has submitted to the Board a Vacation Petition as
<br />completed by Mr. Jeff Gibney, Executive Director, South Bend Heritage Foundation, to vacate the
<br />following alley:
<br />The first north south alley east of South Chapin Street from the north right of way of Napier
<br />Street to the south right of way of West Wayne Street for a distance of approximately 154
<br />feet and a width of 10 feet. All taken from the east side of Lots 11, 12, and 13 of Huston's
<br />Subdivision of Bank Outlot 72, South Bend, St. Joseph County, Indiana.
<br />Mr. Caldwell advised that the Board is in receipt of favorable recommendations concerning this
<br />Vacation Petition from the Area Plan Commission, Department of Economic Development, Police
<br />Department, Fire Department, Engineering Department, Solid Waste. Therefore, Ms. Roemer made
<br />a motion that the Board send a favorable recommendation to the Common Council concerning this
<br />Vacation Petition, subject to any utility easements that may be required. Mr. Powell seconded the
<br />motion which carried.
<br />APPROVAL OF RECOMMENDATION - USE OF "SLOW ' PORTABLES - EWING AVENUE
<br />FROM ERSKINE TO FELLOWS STREET - RILEY HIGH SCHOOL
<br />Mr. Caldwell stated that favorable recommendations have been received by the Division of
<br />Engineering, Police Department and the City Attorney's office, in reference to a request as submitted
<br />by Mr. George W. McCullough, Jr., Principal, Riley High School, to close Ewing Street from Erskine
<br />to Fellows Street, on June 18-19, 1999. Upon a motion made by Ms. Roemer, seconded by Mr.
<br />Powell, the Board approved the use of"slow" portables, and Ewing Avenue will be " handicap only"
<br />parking on tl.e north side from Fellows Street to Erskine Street.
